         completed form are required, to reach the Radio Authority AT LEAST EIGHT
         WEEKS before the proposed launch date of the service. Incomplete applications
         will not be accepted. Each application must be accompanied by a cheque for £200
         made payable to 'The Radio Authority'.

     include any period of time required for test transmissions. We cannot guarantee to
     license you by this date, but we will endeavour to do so, within reason.

           1)      the applicant and licensee (if different) are not disqualified persons
                   within the meaning of that expression as defined in Part II of Schedule
                   2 to the Broadcasting Act 1990 as amended by the Broadcasting Act
                   1996;

           2)      the applicant and licensee have not been convicted during the last
                   five years of any offence under the Wireless Telegraphy Act 1949, the
                   Telecommunications Act 1984 or the Marine &c, Broadcasting
                   (Offences) Act 1967;

           3)      the applicant and licensee have done all they can to ensure that no
                   person who is disqualified from holding a licence by virtue of the
                   Wireless Telegraphy Act 1949 is concerned in the operation of the
                   LRSL;

           4)      the applicant and licensee are mindful of their responsibility to
                   comply with the Authority’s Codes on Programming and on
                   Advertising and Sponsorship;

           5)      the applicant, licensee and anyone concerned in the operation of the
                   LRSL have not been convicted of any offence whether committed in
                   the United Kingdom or elsewhere within the ten years before the date
                   of this Declaration (excluding any spent conviction under the
                   Rehabilitation of Offenders Act 1974 and minor driving and parking
                   offences);

           6)      the licensee is not an undischarged bankrupt, nor has any liquidator,
                   administrator, receiver or similar officer been appointed in respect of
                   the licensee;

           7)      any involvement of a political or religious organisation has been
                   declared;

           8)      the information contained in this application and in any documents or
                   particulars submitted in accordance with it is true and correct to the
                   best of your knowledge and belief.
