     Fill out a Direct Deposit Banking Slip (print off PDF File) and send back via fax to the Old Massett
       office.:
     To Access your Grant once approved: Please forward you Fall Semester Original Transcript (must
       be in a sealed envelope) to the Old Massett office along with a cover letter stating your
       requests.

 Continuing applicants Final Checklist of Documents that must be included in order to be
 considered for a grant:

    Copy of Admission Letter (if transferring to a new school and or a course/registration outline of
       next year’s courses):
    Provide a cover letter stating your requests and future
    To Access your Grant once approved please forward your Fall Semester Original Transcript (must
       be mailed and in a sealed envelope) to the Old Massett office for payment. All post-secondary
       grants will be issued in late February only if all supporting documents have been submitted to
       the Old Massett office.

       in the subject line ( example: post -secondary student – name)
