                             Checklist

  Please make sure all of the following items are submitted for each
 applicant. Only students that have each item submitted by April 30,
         2010 will be considered for the President’s Academy.



   Completed personal data form with all required signatures

   Current copy of applicant’s high school transcript

   All available standardized test scores (ACT, SAT, etc.)

   Teacher reference form completed by a math teacher

   Teacher reference form completed by a science teacher
