                                                     Summary of Extended Day Programs
Submittal of the summary report of Wyoming Bridges extended day programs completes the SY11-12 process for the grant. This information must
be submitted to the Department no later than June 15. 2012, but can also be accepted earlier. Reimbursement for extended day expenditures will
be made upon receipt of these documents, after verification of their completion.

       We would appreciate any information you think might be helpful to others in planning or operating extended day programs. For
  17   instance, if you have encountered problems with hiring quality instructors for extended day programs, how did you resolve this?
       We would like to make available best practice and helpful information to all.




       Isolating and assessing the effectiveness of extended day programs is a challenge. We are interested in learning how the district
  18   assesses the effectiveness of the extended day program on student achievement. Please explain how you evaluate the success

      Program Statistics
Entry Gathers information on student enrollment and completion, as well as other data relative to the operation of your extended
 # day program.
      Math Attendance & Completion Data – Please complete the form with numbers of students identified (ID’d) as needing to
      attend extended day programs for math instruction. Indicate the number of students originally identified, the number who
  1   actually enrolled, and the number who completed extended day instruction in math. Also, provide information on the number
      of students in your math extended day program who met all, some or none of the math goals set forth in their Individual
      Learning Plan.

      Please indicate how many students listed in Questions #1 also attended Bridges extended day programs to receive additional
  2   instruction in language arts.
      Language Arts Attendance & Completion Data – Please complete the form with numbers of students identified (ID’d) as
      needing to attend extended day programs for language arts instruction. Indicate the number of students originally identified,
  3   the number who actually enrolled, and the number who completed extended day instruction in language arts. Also, provide
      information on the number of students in your language arts extended day program who met all, some or none of the
      language arts goals set forth in their Individual Learning Plan.

      Programs are required to incorporate enrichment activities in extended day programs in order to more effectively engage
      summer school students in learning. Here please describe the opportunities you made available to your summer school
 14   students, by grade group, explaining how you tied activities to learning goals, how many children participated, how you
      evaluated the effectiveness of these activities, and your assessment of their impact on student learning.

                                                                Definitions

      Asks about district utilization of transportation for students attending your extended day programs. Select Y if you did provide
 15   transportation or N if you did not provide transportation
      Assessing district ability to make nutritional food available to students attending extended day programs. Select Y if you did
 16   provide snacks or N if you did not provide snacks
      Seeking to find unique and/or best practice information from districts on how they resolved problems they may have
 17   encountered with the operation of extended day programs. We wish to share information with all districts on how problems or
      other challenges might be resolved.
      Please provide information on how your district evaluates the success of students attending extended day programs, thus the
 18   success of the program itself. We are trying to ascertain how districts might be able to evaluate student growth as a result of
      an extended day experience instead of from the regular school day.
      Seeks from districts suggestions on how the Bridges program can be improved in the future, and how the Wyoming
 19   Department of Education could better administer the existing program.

      Program Expenditure Data Report here all expenditures you had for extended day programs. Please provide a record of
      expenditures of the Wyoming Bridges grant funds, as well as expenditures you may have had in excess of those funds from
      other sources, i.e., general funds or Title dollars.
      Report in the first column under "Total Expenditures" all expenditures you had for extended day, regardless of source. Report
      in the second column "Wyoming Bridges Grant Funds" only the Bridges grant funds your district expended for your
      extended day program. The difference from total expenditures versus Bridges grant funds expended will be calculated by the
 20   form and shown in the "Difference" column. We are not only seeking to gather information about expenditures of Bridges
      grant funds for your extended day program, but wish to gather data on the total cost of extended day programs, including
      the expenditure of all funds, regardless of source.

      This question is asked to ascertain the various fund sources districts use to pay for their extended day programs, other than
 21   Bridges grant funds. Total funds reported here should equal funds in the "Difference" column from the prior question.
