Creating Simple Forms on Buzzsaw with Google Docs
You'll need a Google account, it's easy and free. Just goto
http://docs.google.com to set up your account.
Start a new form
From Google Docs
1. File
2. New
3. Form
Author your form
1. Choose "add item" to add more fields
2. Choose a theme (optional)
3. Name the form
4. Add any comments to help the user complete the form ("please use full name"
or "only one name per field")
5. Use the editing tool to create your form
6. Decide if this field is required or not
7. Click Save when finished
Preview your form
1. Fields with an asterisk * are required
Copy the code to embed in a temporary Google site
1. Select "More actions" and embed
Copy the embed code
1. Copy the code
2. Select cancel
Goto Google Sites at http://sites.google.com
We'll quickly create a site on Google sites
1. Select "Create new site"
Give your site a name
1. Use the blank template, or choose a theme
2. Give the site a name
3. Enter the security code
4. Select "Create Site"
Let's get the embed code into the new site
1. Select "Edit Page"
Enter HTML mode
1. Select the HTML button
Paste the embed code
1. Be sure the HTML tab is selected
2. Paste the embed code
3. Select "Update"
Copy the name of the site
1. We'll be pasting this URL into Buzzsaw
Add the form to Buzzsaw
Login to Buzzsaw (you should be an administrator of the site)
1. Find the location that you be placing the form. In this case, the form is an
electronic document transmission agreement. Right click on the folder that you'd
like the form to reside in
2. Select "Add"
3. Select "Link"
Link the form to Buzzsaw
1. Give the Form a Title
2. Paste the URL from earlier into the Address field
3. Select "Finish"
Users should complete the form
1. When other site members are completing the form, they'll be directed to this
link. You can direct them to the link by sending them an email.
2. Make sure the "View" Tab is selected
3. The first 3 fields are required, with an optional "Comments" field at the bottom
4. Users then select "Submit"
The signer is done
This is what the users see when they complete the form
Monitor responses
1. As administrators, we can monitor the list of those who have signed the form
by visiting the form in Google Docs. You can view the responses in summary or
Spreadsheet format

Here are the individuals who have signed the form
Even this response list can be shown in Buzzsaw
1. Copy this link, then add it to Buzzsaw as a link (the same way we added the
form the Buzzsaw). Be sure to set permissions to that this is hidden from other
members in Buzzsaw.
