Early Head Start to Head Start Application Process
Name of Child: ____________will have his/her 3rd birthday on ______. At 3 years old
the child is able to apply for the Head Start program that services 3 and 4 year olds.
Depending on birthday, it makes him/her eligible for possible transition to Head Start
using the: FALL option or SPRING option.
Early Head Start Family Advocate will assist family with completing a Head Start
Early Head Start Family Advocate will collect copies of birth certificate, insurance
(if applicable) and shot record documents from child’s file and attach to Head
Family will provide EHS Family advocate current income documents for a 12
month period (past year tax return or past 12 months from date of application).
Family eligibility will be determined by Early Head Start Family Advocate with
review by Family Services Coordinator.
The application will be submitted to Head Start in time for selection process
depending on option available.
FALL option for children turning 3 years old by December 31. If a Head Start
slot is opened during the fall (August to December) on child’s birthday they are eligible
to transition into Head Start classroom for that school year. Child must be income
eligible child and 3yrs old to transition into Head Start program. If no slot is available
then remain in EHS. If a slot is offered a family can decline the HS slot, have 30 days to
transition to another setting and withdraw from EHS program.
SPRING option for children turning 3years old after December 31. If child is
income eligible, then submit application for consideration in HS selection meeting. If
child is selected for HS program in the August, then child will remain in EHS till end of
spring program and start HS in August. If child is not selected then child will remain in
EHS till end of spring / summer program and assistance will be provided to find other
child care options. Family can decline summer services and withdraw at end of spring.
For a child that is income eligible using the Fall and Spring option, the application will
be submitted to selection committee at Head Start. The selection meetings are in May
If the child is selected, will attend Head Start at beginning of next school year .
If not selected child will continue EHS till end of spring/summer program,

For a child that is not income eligible, the Fall and Spring option do not apply. The
child will receive 30 days of services at after his/her 3rd birthday and the last day
of services for will be ____________. Alternative resources will be given to the
family to seek child care by Early Head Start Family Advocate.
