SUBJECT:         Fiscal Year 2010 Title III Request for Proposals

The Area Agency on Aging (AAA), department of the Houston-Galveston Area Council’s (H-
GAC), is soliciting solicits vendors to provide Health Maintenance, Emergency Response,
Residential Repair, Homemaker, Respite Care, Personal Assistance, Home Delivered Meals, and
Participant Assessment Services for older persons living in the Texas Gulf Coast area. The
funding period for this proposal is Fiscal Year 2010, beginning October 1, 2009 and ends
September 30, 2010. Bids will be accepted by H-GAC until 3:00 p.m., July 28, 2009.

The Managed Care Service Request for Proposal can be found at the Houston-Galveston Area
Council Area Council/ Area Agency on Aging website at www.h-gac.com. At the website click
on the following links: Human Services, Area Agency on Aging, and Aging Request for
Proposal Information. Please download all files related to this proposal. Please review the
vendor handbook and application carefully.

The documents are either in PDF, Excel, or Word formats. You must submit the application
using the required forms in the required software only. The website contains all required forms.
The bid application and its supporting documents must be completed in Word or Excel. Bids not
in the proper format will not be accepted. Bid applications and its documents may not be faxed.
