      10. For Services Leading to Supported Employment:

            o   DARS3406, Services Leading to Supported Employment Time Log,
            o   DARS3495, Vocational Rehabilitation Services Individualized Plan
                for Employment (IPE),and
            o   DARS3431, Extended Services Agreement or other documentation
                that a public agency, private nonprofit organization, natural support,
                i.e., employer, coworker, family member or another appropriate
                resource is going to provide these services.

      10. For Services Leading to Supported Employment:
           o     DARS3495, Vocational Rehabilitation Services Individualized
                 Plan for Employment (IPE),
           o     DARS1612 Career and Community Support Analysis,
           o     DARS1613 Supported Employment Services Plan—Part 1,
           o     DARS1614 Supported Employment Support Plan—Part 2,
           o     DARS1615 Supported Employment Support Summary, and
           o     DARS1616 Job Stability or Job Closure Justification Summary
