GEORGIA CHILD SUPPORT ENFORCEMENT PROCEDURE 320
Completing a Personal / Financial Affidavit
To obtain new or updated information from a non-custodial parent (NCP) which can be
verified and documented in the case record for establishing or enforcing an obligation.
O.C.G.A. §16-10-71 and §19-11-10
All Georgia Child Support offices receiving Title IV-D funds including all private vendors
contracted with the Department of Human Resources to provide child support services.
Required Case Conditions:
NCP needs to be interviewed for any reason; either prior to obtaining a consent order or
for enforcement purposes.
Whenever an NCP is scheduled for an initial or follow up appointment, it is
recommended that they complete a new Personal and Financial Affidavit FormPFA
unless such information has been provided within the last 30 days. If the Agent
believes a new form is needed, ask the receptionist to provide a copy to the NCP,
instructing the NCP to complete as much information as they can on their own.
During the interview, the Agent should review the form with the NCP to be sure they
have provided necessary important data. Ask the NCP to produce their driver's license,
auto registration or proof of insurance card, Social Security card, check stubs, Selective
Service card, etc; in order to verify the information provided.
Do not form any pre-conceived conclusions regarding the NCP; rather, ask questions
and permit the NCP to answer them. Have the NCP sign the form and explain the
penalty for false swearing as defined in the law.

The information provided should be used to determine the NCP's total gross monthly
income available for a new or modified support obligation. (Refer to Procedure 330.)
The information may also be used to determine if accident and sickness insurance for
the NCP's children is reasonably available through the NCP's employer. (Refer to
Procedure 330.6.) Be sure to obtain the insurance company name and policy number.
This information will need to be provided to the CP at the time that health insurance is
available for the child(ren). If necessary, call the employer to obtain or verify
information if pay stubs are not available.
When the agent is unable to obtain or confirm any personal or financial information from
the NCP, a subpoena may be necessary. Explain that a subpoena will be issued and
sent to their employer. The NCP may also be served to appear and testify in court, if
