Uploading Instructions for
Special Education Discipline Data
(Suspensions and Expulsions)
(To Be Done at the Building Level)
Due June 30, 2012
Accessing the Report
• Click on the following link:
• You will encounter a screen that gives you a drop-
down box with the option of selecting SDE, Other,
or Meridian School District. This is the domain
that you will be using, so everyone EXCEPT
Meridian choose “OTHER”. Click on “Continue to

If new to the system, click on “Registration” & follow the directions
carefully. If your password has not been used in the last 120 days, you
will apply for “lost password”.
Download and Fill Out Two Templates:
1) Discipline - Expulsion
2) Discipline - Suspension
(If there are no students to report, you will upload a suspension report
with NULL values and will have to enter all zeros in the expulsion

save as “CSV (comma delimited)” on your

saving, but ignore them. Click on Okay & then Yes

Status will be “waiting for upload” or “started”.
If “started”, this means it has errors. Click on the
blue link naming the report to find out why.
Click on “View row errors”
The problem will be explained in the ‘error’ column.
The name, DOB and nine digit EDUID have to match
exactly for the report to upload successfully.
