            Objective
            To outline the principles and process for isolating SCL plant and equipment, thereby providing a
            controlled and safe work environment where energies and hazards have been eliminated or minimised.


            Scope
            This applies to the isolation of plant and equipment energies and hazards at all SCL operated and/or
            maintained sites.


            Definitions
            ATW Coordinator:       An authorised person who undertakes specific ATW planning and/or work
            coordination roles.

            ATW Form: The document used to track the ATW Process, inclusive of WMS, references to Control
            Guides and other guiding documents and specific approval, planning, scheduling and authorisation
            milestones.

            ATW Process: The approval and authorisation of work by following a structured planning and
            scheduling process that identifies hazards and controls for the management and performance of work.

            Authorised person: A person who has undertaken specific training and assessment and has been
            authorised as per the Authority To Work Corporate Standard – HB# 619258 and Authorisation of this
            document.

            CMMS: Means Computerised Maintenance Management System. This system is operationalised via
            SCL’s Movex Citrix Application.

            Danger tag: A safety sign used as part of the isolation process to signify an isolation point.

            Hazardous energy: Energy that has the potential to cause injury or disease to people if released
            where the impact would be moderate or greater. (As per SCL’s Risk Analysis Tables contained within
            the HS&E Hazard Management Corporate Standard – HB# 692247).

            Isolation Checker: An authorised person who physically checks and verifies plant and equipment
            isolations.

            Isolation Officer: An authorised person who:
             verifies and approves Plant Isolation Control Guides,
             implements the steps detailed within Plant Isolation Control Guides.

            Note: Isolation Officers may also undertake specialist isolation functions and fulfil alternative roles as
                   detailed in the Authorisation Section.

            Isolated: Plant or a location that is safe to access and separated from hazardous energy, due to the
            implementation of the following actions:
             activation of isolation points,
             release of stored energy,
             hanging of danger tags,
             securing of locking devices as required.

            Isolation point: A mechanical device that physically blocks/prevents:
             the release of gases / liquids / solids,
             the transmission or accumulation of hazardous energy.

            Locking device: A device that uses a positive means, such as a lock, to hold an isolation point in the
            safe position and prevent the inadvertent operation of a plant item or release of hazardous energy.
            Included are blank flanges and bolted slip blinds.

            Plant Isolation Control Guide (PICG): The document used to track the Plant Isolation process,
            inclusive of a documented group of forward and reverse isolation points and to assist the user in the
            selection of consistent control measures associated with the isolation of plant and hazardous energies.

            Plant protection point:     A device or plant item that is required in a certain status for
            operational/maintenance reasons, and not for the purpose of isolating a hazardous energy.

            WMS: Means Work Method Statement

            Note: Refer also to the HS&E Hazard Management Corporate Standard – HB# 692247.


            Responsibilities
            The Corporate ATW Committee as per the Plant Isolation Principles - System Maintenance Section) is
            responsible for managing plant isolation process changes, audits and system reviews as detailed
            within the Authority to Work Corporate Procedure – HB# 619259.

            Specific responsibilities of authorised persons that apply throughout the Plant Isolation Process and
            Special Isolation Procedures are detailed within the Plant Isolation Corporate Procedure – HB#
            569944.


            Controls
            Plant Isolation Principles
            Plant Isolation Process
            Any plant isolation process, inclusive of special isolation procedures, is to be undertaken as a sub-part
            of the ATW Process. Refer to the Authority to Work Corporate Standard – HB# 619258.

            The plant isolation process, inclusive of control guide development and hanging of danger tags and
            locking devices where required, is to be undertaken for any isolation of hazardous energy, apart from
            those specific scenarios that are identified as Special Isolation Procedures. These special isolation
            procedures include alternative or additional requirements to that detailed within the plant isolation
            process. Refer to Special Isolation Procedures Section.

            The plant isolation process, including the development of a plant isolation control guide is to be
            documented in accordance with the Plant Isolation Process Section.

            Protection from Plant Energies and Hazards
            The plant isolation process must involve two authorised persons at each of the three key isolation
            stages as shown below. This requirement does not prevent two persons from completing all three
            stages as long as they are both authorised for the role they are required to perform.

            Where possible, the isolation process and the checking process are to be undertaken independently.
            That is, a full isolation process, followed by a full and independent walk-through check. This includes
            all forward isolation sequences, including any sequence changes undertaken as part of plant isolation
            suspension process.

            Where an independent check reduces the level of safety of those isolating or checking, or where the
            location of the isolation is remote, the isolation and check may be done in real time. That is, the
            isolation, tag out, (and lock-out if applicable) is made and checked by the second person as it is
            undertaken.

            Control measures for performing the isolation, which are identified within a plant isolation control
            guide, must be implemented by the person who performs the isolation.

            Isolation methods, identified within specific plant isolation control guide forward and reverse steps,
            must be implemented by the person who performs the isolation and subsequently checked by the
            person undertaking the checking role.

            Chosen isolation methods must ensure that hazardous energy is isolated.

            Once isolated, each isolation point must have a danger tag hung at that point by the isolation officer.

            The securing of a locking device must form part of the isolation method for sources of electrical supply
            above extra low voltage where these sources are being isolated for the purpose of performing
            electrical work.

            Note: Specific requirements regarding the securing of locking devices for electrical supplies are
                  included in the Plant Isolation Corporate Procedure – HB# 569944.

           No person is permitted to change the status of an isolation point or a plant protection point unless they
           are an authorised person for that plant and they are doing so in accordance with this Corporate
           Standard.

           No person is permitted to place or remove a danger tag, locking device or isolation lock unless they
           are an authorised person and they are doing so in accordance with this Corporate Standard.

           As per the above requirements SCL views unauthorised or deliberate operation of isolation points or
           removal of danger tags very seriously. Accordingly, specific disciplinary processes will be initiated if
           such an incident was to occur.

           If a danger tag, locking device or isolation lock is identified as having separated from a plant item or is
           found in a random location on site, the issue is to be immediately reported to an on-site isolation
           officer who can instigate an investigation as appropriate.

            Note: Specific contingency requirements for scenarios involving lost keys, locks, unavailable key
                  holders, etc, are included in the Plant Isolation Corporate Procedure.

            Special Isolation Procedures
            Special isolation procedures include:
             Master Isolation processes,
             High Voltage Distribution Isolation processes,
             Multiple Site / Principal Contractor Isolation processes,
             Specific Site and Local Alter Isolation processes that are included within the Register of Approved
              Site and Local Alter Isolations – Hummingbird # 692099,
             Own Isolation processes that are undertaken on plant that is included within the Register of
              Approved Own Isolations – Hummingbird # 692101.

            Special isolation procedures must only be used for the scope of which they are intended and/or for the
            plant that has been approved.

            Specific definitions and requirements for special isolation procedures, and the above listed registers,
            are to be maintained within the Plant Isolation Corporate Procedure – HB# 569944.

            Authorisation
            A person can become authorised in a number of roles (ATW Coordinator, Isolation Checker, Isolation
            Officer, etc), at a particular SCL site.

            If a person is an authorised ATW Coordinator and an authorised Isolation Checker having successfully
            completed the relevant training courses that relate to the scope of the work activity and the specific
            plant area, they may fulfil both roles as part of a single ATW Process.

            A person who is not a SCL employee can become authorised in any plant isolation role provided they
            have undertaken the required training and additional on-site practical experience necessary for that
            role.

            If an Isolation Officer has practical plant isolation experience and plant knowledge at the site, inclusive
            of special isolation procedures, they may fulfil the role of a Plant Isolation Coordinator.

            If an Isolation Officer, also authorised as a Plant Isolation Coordinator, has experience in overseeing
            isolation processes during unit outages or implementation of the master isolation process, they may
            fulfil the role of a Master Isolation Officer.

            For specific detail regarding Plant Isolation Coordinators, Master Isolation Officers, requisite training
            and additional requirements for personnel involved in plant isolation processes, refer to the Training
            and Competency Requirements Section.

            Plant Isolation Control Guides
            Verification and approval of each Control Guide is to be undertaken as part of plant isolation planning.
            The verifier and approver of a Control Guide cannot be the person who prepared the Control Guide
            and must be an authorised person having successfully completed the relevant training courses that
            relate to the scope of the isolation and plant area.

            Verification and approval must be provided for each Control Guide. This verification and approval for
            Control Guides can only remain effective if the Control Guide is unaltered during preparation and for a
            maximum period of 12 months.

            This section also applies to Master Isolation Control Guides that are developed as part of special
            isolation procedures, refer to the Plant Isolation Principles – Special Isolation Procedures Section. For
            specific information relating to Master Isolation processes, refer to the Plant Isolation Corporate
            Procedure – HB# 569944.

            System Maintenance
            Managing and reviewing the Plant Isolation Process, its related systems and this Corporate Standard,
            is the principle function of the Corporate ATW Committee. This will be undertaken on an as needs
            basis and at periods not more than two-yearly.

            Specific auditing and system maintenance requirements are to be undertaken as prescribed within the
            Authority to Work Corporate Standard – HB# 619258 and Authority to Work Corporate Procedure –
            HB# 619259.

            Incident and Non-Compliance Management
            Health, safety and environmental incidents or breaches of this standard that occur as part of the Plant
            Isolation Process will be recorded through the Heath, Safety and Environmental incident reporting
            process, refer also to the Hazard and Incident Reporting Corporate Standard – HB# 796990. Details
            of these incidents will also be tabled and discussed as part of the Corporate ATW Committee forum.

            Plant Isolation Process related non-compliances and audit / review outcomes will be recorded in SCL’s
            Audit Response Database and managed as part of the Corporate ATW Committee functions.

            Modes of Operation
            The default mode of operation to initiate the Plant Isolation Process and develop or review an existing
            Plant Isolation Control Guide will be via the ATW Application. That is, the ATW Application must be
            used where possible.

            In the event of a system outage or in the absence of a CMMS system, Plant Isolation Control Guides
            and associated documentation is to be developed or amended as per the contingency requirements
            detailed within the Authority to Work Corporate Procedure – HB# 619259.

            Plant Isolation Process
            The Plant Isolation Process represents only one sub-part of the ATW Process and is primarily tracked
            via a Plant Isolation Control Guide. This document serves as the interface between the personnel
            involved with the work and the relevant isolation controls that are required to ensure safety for
            personnel, plant and environment. It is also the document that formally tracks the status of a plant
            isolation during its entire use.

            The plant isolation process, including the development of a plant isolation control guide is to be
            documented in accordance with Plant Isolation Corporate Procedure – HB# 569944 and primarily the
            Plant Isolation Process Flow Chart contained in Attachment 1 of that document.

            Plant isolation suspension processes as listed below must also be undertaken as prescribed in the
            Plant Isolation Process Section previously.
             Alter isolation,
             Testing / Commissioning,
             Availability.

            Where other SCL Corporation Standards and Procedures prescribe ways for managing hazards that
            may be part of a Plant Isolation Process these requirements must also be followed.

            In specific circumstances where it is identified that the Plant Isolation Process cannot be implemented
            or ought not to be implemented in the interests of people, plant and the environment, the contingency
            process is to be followed and authorised as per the Authority to Work Corporate Procedure – HB#
            619259.

            The following table highlights the specific accountability requirements for the ownership and
            implementation of the various Plant Isolation Processes:

              Accountability for Plant Isolation Processes

                    General Manager         Accountable for ensuring that an appropriate plant isolation system is available at
                    (O&M Services)          all times.
                                            Accountable for ensuring that the plant isolation system is communicated to and
                                            implemented by Site Management.
                                            Accountable for ensuring that the Corporate ATW Committee monitors plant
                                            isolation system issues.
                                            Accountable for ensuring additional SCL resources are called upon and
                                            consultation undertaken, to review, update and maintain the plant isolation system.

                     Site Responsible       Accountable for ensuring the plant isolation system is implemented at the site,
                         Manager            inclusive of:
                                            ·    ensuring all persons entering the site are aware of and understand relevant
                     (SCL Operated and           requirements of the plant isolation system,
                      Maintained Sites)     ·    ensuring all persons undergo competency training and are authorised for any

            Training and Competency
            All plant isolation related training courses will be developed, delivered and maintained as per the
            Training & Assessment Guiding Principles Policy, and the Corporate Standard for Training and
            Assessment – HB# 693807.

            Induction, pre-requisite and external training requirements for those involved in plant isolation
            processes are to be undertaken as per the Plant Isolation Corporate Procedure – HB# 569944 and the
            Authority to Work Corporate Standard, Attachment 1.


            Review
