         APPLYING TO THE DOCTOR OF PHYSICAL THERAPY (DPT) PROGRAM
                     UNIVERSITY OF EVANSVILLE STUDENT

1.        Save a copy of the application for completion.

All portions of the application must be typed; facsimile copies will not be accepted.
Sign the completed application.

2.        Print two copies of the Recommendation for Admission form.

Follow the instructions provided on the form. One of the recommendation forms must be
completed by a physical therapist who you have worked with either on a paid or volunteer basis.
The second recommendation must be completed by a faculty member from whom you have taken
at least one course.

3.        Submit transcripts from all universities attended. A minimum 2.75 prerequisite science
          and math GPA is required to be considered for admission. A 3.0 overall grade point
          average is recommended. A “C-” in all coursework is required.

         Applications for summer 2013 will be accepted July 1 through October 15, 2012
          (postmarked)
         Qualified applicants will be invited for an on campus interview
         Offers of admission will be made based on overall and prerequisite science and math
          grade point averages, application assessment, interview assessment, and extra points
          earned for coursework successfully completed at UE
         Once forty positions are filled a wait list will be compiled
         Applicants on the wait list will be ranked by scores received for science and math
          prerequisite and overall grade point averages and application and interview assessments
         If an accepted applicant declines our offer of admission, the applicant ranked first on the
          wait list will be notified. The wait list will be maintained until June 1, 2013
         Application materials and information regarding admission criteria and the selection
          process may be accessed through the Department of Physical Therapy's Web site at:
          www.pt.evansville.edu

Processing of application materials will not begin until all of the following materials are
received by the Department of Physical Therapy
        1. DPT Application, stamped by the Office of Student Accounts confirming that the
            $30.00 fee has been paid, and two recommendations for admission
        2. Transcripts from ALL universities attended (copies of transcripts may be obtained
            from the Office of the Registrar
