REQUIRED ATHLETIC REGISTRATION DATES
This document will explain what is required in order to participate in a sport at FGR. Each year
we need to collect all the necessary athletic paperwork (and fees) in order to be in compliance
with the MHSAA (Michigan High School Athletic Association) and CHSL (Catholic High
School League). This is done through our athletic registration process in August (for ALL
sports-ALL 3 seasons). All the information you need to know to complete this process is listed
below as well as at www.fgrathletics.com under the TAB at the top of the main page titled
“FORMS/ INFORMATION”. Please carefully read through all the information. If there are
ANY missing documents or signatures then the registration process can not be completed. If the
process is not completed the athlete can not try-out, practice or play.

You must attend ONE session:
Monday, August 68-10am and 5-7pm
Registration for FOOTBALL players only: 1pm
Tuesday, August 78-10am and 5-7pm
Wednesday, August 8 8-10am and 5-7pm (all other fall sports start
practice/try-outs this day)
In the past we have been able to secure a doctor to come in and do physicals during the evening
sessions for anyone who needed it ($20). Please check the athletic website for updated information.
We will post details once we confirm whether or not the doctor is available.
Please note if we secure a doctor it is on a first come, first serve basis.
If your insurance will not cover a physical before the required timeline (April 15, 2012 or later) than
please seek Urgent Care Centers. If you don’t play a fall or winter sport, then please turn in all
OTHER required forms and turn in your physical once completed.
LAST SESSION (ALL athletes must register by this date if they missed the earlier
August dates-no exceptionsthis includes ALL winter and spring athletes)
Wednesday, August 29 from 7-9pm. (school is in session)
Turn in paperwork directly to the ATHLETIC OFFICE (in the gym)
Remember: Do NOT mail forms in to the school. The main office and coaches will not accept athletic
paperwork. If you are planning on playing a sport next year you must attend one of these sessions to
turn in all paperwork.
ALL 4 Forms (5 for football players) are due to Athletic Department during registration day
to be cleared to try-out/practice (click on links to pull up forms).
*Please note that if you are missing any of these four documents (missing parent and/or student
signatures are also considered incomplete) you will NOT be able to practice OR try-out.*
1. MHSAA Physical Form
Health Appraisals get turned in to the MAIN office, not the
athletic office. Note: If you are using your health appraisal form
as your physical please make a copy and attach it to the MHSAA
forms. You must still complete both the front and back and sign
all required areas of the MHSAA form
2. Assumption of Risk (all athletes EXCEPT FOR FOOTBALL)

Required FEES (All fees must be on separate checks):
1. Athletic Participation Fee
(Must be paid at the beginning of EACH season before the 1 st contest) Payment Plan Worksheet
for Participation Fees is available if you can not pay the full amount at beginning of the season.
($50 deposit at start of season is required for all payment plans)
Checks payable to FGR $175 for all sports and all levels
2. FGR Athletic Team Warm Up
(Order online at www.fgrathletics.com by the established cut off date)
3. Athletic GR Tie for Game Day Dress (should be worn with khaki pants and white oxford shirt)
Check payable to FGR for $20
SPIRIT STORE WILL BE AVAILABLE TO PURCHASE ITEMS DURING ATHLETIC REGISTRATION
4. Check for Family Pass (Can be purchased at any time!)
