Rental program unit owners will be assessed a $40 cleaning fee if the unit is found dirty after access.
*Room block will appear as Out of Order Days on hotel rental program owner’s monthly distribution statement.

It is the owner’s responsibility to ensure the all authorized persons understand and adhere to the
operational procedures and regulations of the Ala Moana Hotel.
I DO NOT want to reserve my unit as “Out of Order Days”. All showings or entries will be confirmed when a hotel guest is not
occupying the unit. Access into the unit is subject to availability, with no guarantee. The hotel will not room move an in-house
guest or reservation to accommodate realtor showings or inspections.
I authorize the above Realtor(s)/Property Manager(s) FULL MANAGEMENT of my unit, including the ability to schedule showings or
confirm reservations directly with the Owner Relations Office without my individual approval. I also acknowledge and have advised
the following information to my appointee:
ALL AUTHORIZED ENTRIES REQUIRE AN INDIVIDUAL & COMPLETED ACCESS FORM OR RESERVATION FORM.
REALTOR SHOWINGS OR INSPECTIONS CANNOT BE COMPLETED IF UNIT IS OCCUPIED OR VACANT-DIRTY.
VERIFICATION OF PHOTO ID IS REQUIRED FOR ALL KEY ISSUANCES.
Owner Relations office hours are Monday-Friday, 8:00 a.m. – 5:00 p.m. If access forms are received outside of business hours, the
request will be processed the next working day and is subject to availability.

Regulations for SHOWINGS / LISTINGS / OPEN HOUSE:
1. No SIGNAGE of any kind will be allowed on the door of the unit or anywhere on the hotel property.
2. Only AUTHORIZED PERSON noted on this request form will be given key access. ALL CHANGES MUST BE SUBMITTED IN WRITING.
3. KEYS MUST BE RETURNED to the Front Desk when unit entry is completed. If key is not returned, owner may be subject to $40 check-out fee.
4. The realtor/inspector is responsible for any cleaning necessary to ensure the unit is ready for the next guest use. If the unit is found dirty, owner will
be notified and assessed a $40 cleaning fee.
5. The realtor/inspector will be responsible for any damages or missing items while in possession of the issued key and it will be the owner’s
responsibility to collect for such from the realtor/inspector.
