Job Description Name of Role Community Support Worker Purpose of Role It is the role of the Community Support Worker role to provide high quality support assisting customers to continue to live in their own home, and enable participation in social and community activities. Key Result Areas To provide positive support to meet customers bespoke needs on an individualistic basis, with respect, empathy and sensitivity. To ensure all the customers’ Social Services assessed needs are met in line with their care plan. Support customers flexibly in response to their changing needs and aspirations. To have a good working knowledge of Microsoft Word, Excel and the use of the internet. To work using a person centred framework for practice. Promote independence. To support customers to access social and community activities of choice. Support customers to live full lives and develop a range of relationships. To support customers to facilitate housekeeping activities where possible. Support with shopping, telephone calls, budgeting, leisure and recreational activities, reading and writing letters. Contribute to multidisciplinary action plans as required, for example, health action plans, person centred plans. Contribute to reviews as necessary. Participate as key member within the Self Direct Support team. To report and document as required by the individual customers. Ensure identification of risk and assist in effective risk management. Communicate effectively with customers, team members, professionals and stakeholders. Ensure professional boundaries and best practice maintained at all times. Work within legislation, guidelines and Self Direct Support policies and procedures. Work to an agreed rota. Once shifts agreed ensure absences or change of circumstances are reported in timely manner so cover can by organised. To undertake training as required To work in accordance to the General Social Care Council’s Code of Practice for Social Care Workers Scope The Community Support Worker is responsible to their immediate operations manager and registered manager. Person Specification Community Support Worker Essential Requirements Desirable Requirements Education / NVQ 2 Care or a willingness GCSE in English or qualification to work towards equivalent requirements achievement. A willingness to complete the Common Induction Standards within agreed timescale. Person Centred Planning and a willingness to undertake training.
