Under the supervision of the Tuberculosis (TB)/Refugee Program Manager and Case Registrar and with clinical direction
from the Chief of Disease Control & Medical Epidemiology, the Data Entry Clerk is responsible for assisting the TB Case
Registrar/Clinic Scheduler. Duties will include data entry, scheduling patient appointments, answering the telephone and
assisting with medical records.
TASKS & RESPONSIBILITIES: %
1. Assist in performing data entry, electronic data processing and data quality control activities related to the 30
maintenance of the electronic TB data collection and/or record systems, including the entry of patient
registration, encounters and appointments.
Assist with data entry, cleaning, tabulation and reporting of program mass screenings.
Assist with database creation and management, data entry, data extraction, data tabulation and report
creation and distribution of requested TB/Refugee Program data.
Assist with maintenance of the TB program patient appointment system, scheduling patients and entering
DOT/DOPT and contact investigation data.
Assist in the completion of the DSHS Refugee Health Assessment E-Share form.
2. Assist in medical records. 30
Retrieves files and ensures medical records are available for patient care.
Faxes, files lab reports and other medical forms.
Purges and processes inactive records with appropriate entries in the INSIGHT computer system and
preparation for scanning.
Follows approved medical record policies and procedures, including those dealing with patient
3. Put patient records/charts together in preparation for TB chest clinics. 15
4. Serve as clerical support for TB Chest Clinic when needed. 15
5. Assist with the pickup of county mail and TB medications from the City of Houston TB Control Program. 5
6. Other duties as assigned including special tasks involved in responding to an emergency event. 5

UPON ENTRY TO THIS POSITION, AN EMPLOYEE NEEDS THE FOLLOWING KNOWLEDGE, SKILLS, AND

High School Diploma or GED required.
LICENSE(S) AND/OR CERTIFICATION(S) REQUIRED:
Valid Texas Driver’s License required.
EXPERIENCE REQUIREMENTS: (Amount and Type)
One year experience performing data entry, preferably in a healthcare related setting.
COMPUTER PROFICIENCY: (Be specific as to the amount and type of software and/or hardware knowledge required and the proficiency level
of skills required, i.e., basic, intermediate, or advanced)
Intermediate knowledge regarding the operation of a computer and data entry required.
SPECIAL SKILLS AND/OR ABILITIES:
Ability to interact effectively with the community and individuals and communicate their needs to professional staff.
Must be versatile and have the ability to perform job without constant supervision.
Reliable transportation is required for local travel.
Ability to work well with others.
Able to organize materials, make reliable judgments, and draw conclusions from verbal discussions and written data.
Must be punctual, self-starter, dependable, mature, flexible, responsible and have the ability to work well with others.
Ability to speak English fluently. (Bilingual English/Vietnamese is a plus and is preferred.)
