Contractual The Subscriber and T-Mobile hereby agree on a modification of the term of the Subscriber Contract, which is hereby
stipulations extended by 24 months if the Subscriber purchased a device at a discounted price in connection with the conclusion of this
Amendment; otherwise the Subscriber Contract is extended by a fixed term determined in the Frame Contract. The
Subscriber undertakes to observe the Minimum Monthly Payment obligation, the amount of which is specified in the Annex
hereto, for a period of 12 months. The period for observing the Minimum Monthly Payment obligation commences on the
date of the first Statement of Services issued after the date when the new fixed term of the Subscriber Contract started to run
(this period shall be extended by the period during which the provision of services is suspended).
The amount of the contractual penalty and details regarding the Minimum Monthly Payment are specified in the Minimum
Monthly Payment Terms and Conditions. By signing this Amendment, the Subscriber confirms that he/she has read, and
agrees without reservations to, the General Terms and Conditions of T-Mobile Czech Republic a.s. for Business Customers
in force or, as the case may be, the General Terms and Conditions of T-Mobile Czech Republic a.s. for Consumers in force,
depending on the position2) in which the Subscriber enters into this Amendment (the “General Terms and Conditions”), the
Terms and Conditions for Processing of Subscribers' Personal, Identification, Traffic and Location Data in force, the Price
List of Services in force and by the conditions of other activated services. The Subscriber undertakes to read the terms and
conditions of all services that he/she activates throughout the term of the Subscriber Contract; the terms and conditions of
the activated services shall form integral part of this Subscriber Contract upon the date of activation of the particular service.
In the event the Subscriber refuses or repeatedly fails to collect the ordered phone, notebook, accessory or another device
by means of which the Subscriber can use the T-Mobile services (the “Device”), T-Mobile shall store the Device for 12/24
months following the extension of the Subscriber Contract. Should the Subscriber fail to collect the Device within this time
limit at T-Mobile's registered office, the Subscriber hereby grants his/her consent to donating the Device to charity. This shall
be without prejudice to the validity and effect of the Subscriber Contract.
Information about the current setting of the agreement or, as the case may be, disagreement with the processing of data in
accordance with the Terms and Conditions for Processing of Subscribers' Personal, Identification, Traffic and Location Data
is available in the My T-Mobile application or can be obtained from the Customer Centre. The Subscriber may change the
setting of his/her agreement or disagreement with the processing of data in accordance with the Terms and Conditions for
Processing of Subscribers' Personal, Identification, Traffic and Location Data free of charge at any time either in the My
T-Mobile application or by contacting the Customer Centre.
This Amendment comes into force on the date it is signed by the parties. The Amendment comes into effect on the date
when the new term of the Subscriber Contract is set up in T-Mobile's internal systems, though no later than within 30 days
following the day when it is received by T-Mobile.
This Amendment shall be without prejudice to the other stipulations arising from the Subscriber Contract. The agreed new
term shall commence on the effective date of this Amendment or, as the case may be, on the day following the date of expiry
of the original term (in the event that the Contract was concluded for a fixed term). In the event that T-Mobile does not set up
the modified term of the Subscriber Contract in its internal systems within the aforementioned time limit, T-Mobile shall be
deemed to have withdrawn from the concluded Amendment. T-Mobile shall inform the Subscriber that the modified term of
the Subscriber Contract has been set up or that it has withdrawn from the Amendment.
Annex 1 forms integral part of the Amendment.

deemed consumer in the terms of Act No. 40/1964 Coll., the Civil Code, as amended, and Act No. 634/1992 Sb., the Consumer Protection Act, as amended. The General Terms and Conditions of
T-Mobile Czech Republic a.s. for Business Customers are part of the Subscriber Contract of a Subscriber who enters into this Agreement in a position when he/she is not deemed consumer in the
terms of Act No. 40/1964 Coll., the Civil Code, as amended, and Act No. 634/1992 Sb., the Consumer Protection Act, as amended.
