The Performance Management process is inclusive of three forms:

Form 1 - The Performance Planning and Appraisal Report is designed to record
the performance agreement between the employee and supervisor by outlining
the performance objectives to be achieved during the planning period.
Additionally, it is designed to assist in making an objective and factual evaluation
of the performance of the employee by comparing actual performance against
the performance requirements for the job.


Form 2 - The Competency Checklist.

Form 3 - The Employee Development Plan which provides for a summary of the
employee's specific development needs and activities identified during the
performance review process.

                                                     INSTRUCTIONS
Please review before completing the following form (Form 1 - Performance Agreement).
This document includes a 'how-to' for:
•    Inserting and deleting an objectices to be achieved
•    Inserting and deleting a 'key results area'
•    Calculating the ratings score
•    Printing

Inserting an "Objectives to be achieved"
1. Right click the row number of the row to be copied.
2. In the menu box that appears click "Copy".
3. Right click the row number below.
4. Click "Insert Copied Cells".
5. The inserted row will appear.
6. Press the 'Esc' button on your keyboard.

Deleting an "Objectives to be achieved"
1. Right click the row number of the row you would like to delete.
2. Click "Delete".
PLEASE NOTE: Once you have entered your performance information into this report, delete any blank/empty (i.e.
unfilled/unused) rows of "Objectives to be achieved". Blank/empty rows will altar the calculations of the rating score.

Inserting a "Key Results Area"
1. Highlight the row numbers for the entire "Key Results Area" Section.
2. Right click the highlighted area.
3. Click "Copy".
4. Right click the row number of the row below.
5. Click "Insert Copied Cells".
6. Press the 'Esc' button on your keyboard.

Deleting a "Key Results Area"
1. Highlight the row numbers for the entire "Key Results Area" Section.
2. Click "Delete".

Calculating the Rating Score
1. Enter the percentage in Column [B] - % of Job .

2. Enter the performance rating in Column [D] - Performance Rating for each Objective to be
   achieved.

IMPORTANT NOTE: Column [E] - Average KRA and Column [F] - Score for KRA will automatically
generate the appropriate figures. Do not delete or format these cells.

How to Print
1. In the 'Print' dialogue box, find section 'Print what'.
2. Click 'Entire workbook', then click 'ok'.

              Consistently Exceeded - The level of performance achieved was well above what was agreed in the
              Performance Agreement and the performance was consistent. In this case ‘consistent’ should be
3.50 - 4.00   judged over a period of time. It is therefore unlikely that employees will achieve a ‘4’ rating in respect of
              performance during only one year. Employees will need to achieve ‘3’ ratings over time before a ‘4’
              can be justified.

              Exceeded – The level of performance achieved occasionally exceeded what was agreed in the
2.50 - 3.49
              Performance Agreement.
              Succeeded – The level of performance achieved matched what was agreed in the Performance
1.75 - 2.49
              Agreement.
              Not met – The level of performance was below what was agreed in the Performance Agreement. Not
1.00 - 1.74
              eligible for increment.
