Vacancy Requisition Form - Guidance
The Vacancy Requisition Form combines together all the information that is necessary in order to set up the vacancy details within the recruitment process. The
requisition is the authority to proceed with the vacancy.
Some of the information supplied to complete this form is required to complete the Position Creation Form or Position Amendment Form (if applicable). The Vacancy
Requisition Form must be approved by the Divisional HR and Finance Manager plus the budget holder/Divisional Director depending on local agreement.

Section 1 - Position Details and Advertising (to be completed by Manager)
Position No This is an ESR system generated number. If you don't already have a list of position numbers for your team your local HRO will be
able to provide the number(s).
No of vacanct posts The number of vacancies / the number of people you need to recruit.
Position Title This is the job title and should match exactly the title on ESR
Division Select the relevant Division from the drop down menu
Organisation: This is the department or team name. Please select the correct organisation from the drop down hierarchy. In ESR every organisation
(department) is linked to a cost centre and is where all staff costs are charged. Please do not select Divisions or Sections
Location This is the address at which the postholder will be based
Band Select the appropriate band for the post from the drop down list
Full/Part Time Select whether the vacamcy is part time or full time
WTE Please insert the whole time equivalent for the vacancy
Hours/sessions Please enter the number of hours/sessions to be worked
Shift Pattern If applicable, please indicate the shift pattern
Appointment Type Please select the appropriate appointment type
Contract length & reason If the vacancy is not permanent, please enter start date and end date of contract and a give a brief explanation
Summary of role Please give a brief summary of the role
Reason for vacancy Please give a brief explanation of the reason for the vacancy
