                            CONVENTION ACCESS SERVICES

If you require access services, please check any needed service(s) and provide your name,
phone number, and email address. Please return this form with your registration. To
guarantee Access Services, your request must be received at least four weeks in advance of
the Convention.

              the session.

In addition, sign language interpreters will interpret all Keynote Presentations, the President’s
Reception, and the CAPED Business Meeting.
