Mini-Grant Criteria and Application Form
Health Care Home Certification
Focused on transformation and administrative support for HCH in Clinics
Project to be implemented between June 1, 2010, and Nov. 30, 2010
The purpose of these mini-grants is to assist clinics in completing the final implementation of the
HCH standards or administrative work needed for Health Care Home certification.
We welcome projects that describe proposals to finalize the transformation component that
supports the clinic implementation of health care home standards. Examples include:
contracting with a facilitator or coordinator to assist the HCH team to finalize and
implement a component of the standards such as a workflow or procedure;
completing the acquisition/development of an electronic registry to support patient
quality improvement and meeting HCH standards;
creating a care plan document or workflow and/or integrating it in the EMR;
developing the HCH quality team that includes patients; or
implementing the HCH payment methodology
Additionally, administrative support proposals to refine or create documents that support HCH
implementation in your clinic will be considered. Examples include
hiring temporary administrative staff to prepare documents such as updating clinic
workflow or training documents or policies/procedures to meet HCH standards;
preparing for the HCH certification site visit including completion of application
documents; or
other priority projects related to the HCH certification process.
At least 12 project proposals will be funded to clinic/ organizations in the process of implementing
health care homes.
Grant recipients will be expected to provide a brief summary report upon completion of the project,
to have submitted the HCH certification application and the HCH assessment tool and have
scheduled a HCH certification site visit by November 30, 2010. Grant recipients may be asked to
present their project at future statewide health care home learning collaborative events.
Development of health care homes in Minnesota is part of the ground-breaking health reform legislation
passed in 2008 and is coordinated by the Minnesota Departments of Health and Human Services. The
Health Care Home Section at the Minnesota Department of Health has contracted with MAPF, MAFP,
and MN-ACP to provide mini-grants for up to twelve clinics/systems with a maximum of $5,000 per
clinic/system. Mini-grant applications should be sponsored by clinics/systems of care that are
considering certification as Health Care Homes in Minnesota. The clinic/system should also complete (if
they have not already) a HCH letter of intent.

Criteria for selection of mini-grant awardees:
1. Mini-grant HCH team will have more than 50% of the HCH standards and criteria

2. The Mini-grant HCH team members will commit to the following activities:
Engage their clinic/system in clinic planning, technology use/integration and/or
administrative support for HCH implementation.
Submit a completed HCH certification application and certification assessment tool
and schedule a HCH certification site visit by November 30, 2010.
Establish a dissemination process of what they learned within their team/clinic/system
Lend expertise by presenting or facilitating at a 2010-2011 Learning Collaborative event.
Serve as a resource person/mentor for other potential HCH teams in your region.
3. Mini-grants will be selected by a collaborative group of MDH / Medical Association staff.
Criteria include:
composition of the clinic/system team;
type of clinic;
location(s) of clinic/system team;
number of standards currently implemented and ability to achieve a successful
certification site visit in November 2011.
rationale for participation and plan for implementation within clinic/system.
plan for future spread of HCH in the clinic / system.
