A. To qualify for CSF membership this semester, follow these guidelines:
    1.   You must earn a minimum of 10 points from last semester's grades.
         a. The first 4 points must be from LIST I (unless you are a senior applying for membership in February or June).
         b. The first 7 points (including the four points described in (a) must be from LISTS I and II.
         c. The remaining points may come from any LIST (I, II or III).
    2.   You may not use more than 5 courses to qualify.
    3.   CSF points are granted as follows:
         a grade of A = 3 CSF points.
         a grade of B = 1 CSF point. (Note: a grade of B in an AP/Honors course earns 2 CSF points, an A earns 4 CSF points
          **you may only use 2 courses with this designation in the specified category.)
         a grade of C = 0 CSF points.
         a grade of D or F in any course, even in one you cannot use to qualify, disqualifies you from membership at this time.

B. Also remember:
         1.   Life Membership shall be granted to those students who qualify in 4 semesters, and one of those semesters must come from the senior
              year. Students have 6 semesters starting their sophomore year to achieve life membership.
         2.   Associate Membership will come from the second semester of the ninth grade year.
         3.   Membership is based on academic work from the previous semester. Courses may not be repeated and in special circumstances, such as
              an impacted schedule, summer course offerings may be used.
         4.   Students must be an active member during their high school experience by attending meetings and completing proper documentation each
              semester. It is the student’s responsibility to make sure they have an appropriate schedule meeting CSF qualifications.
         5.   Students who achieve life membership will receive CSF pin, diploma seal and honored graduation status.
         6.   Courses are divided into designated lists and are found on the back of this form.
         7.   Parent signature on this form validates your status as an active CSF member.
