III. PLAN OBJECTIVE
The Emergency Plan for this department was created to define the responsibilities and procedures to be followed by staff in effectively responding to an emergency.
IV. OVERVIEW
This plan was developed for staff located at AC-50. A copy of the plan, as well as any future updates, will be made available to all staff at AC-50. New hires will be provided with this document as part of their employee orientation session. Every staff member at AC-50 should be aware of the following:
The First Aid/Disaster Emergency Supplies are located at under the main workstation labeled NTWOPNS01 and NTWOPNS02.
Staff members (listed on pg. 2) have been designated as the department's Emergency Coordinators. Their primary task is to assist in the safe and orderly evacuation of their assigned units including the following: o Ensure everybody vacates the building. o Account for all personnel in the unit following the evacuation. o Communicate with UCPD/other emergency personnel as necessary, and check in with the OAAIS Facilities Coordinator and/or your departmental manager or director
During a disaster, stay calm and follow directions given by your Emergency Coordinator. Your Emergency Coordinator will provide the necessary instructions for your safety.
V. EVACUATION PLAN & ROUTE
A. General Considerations:
Circumstances which might necessitate evacuation:
Fire Bomb threat Earthquake with significant structural damage (evacuate only after shaking stops) Other situations which would mandate evacuation to avoid injuries to staff
People with Disabilities:
Persons with disabilities shall proceed if possible to nearest stairwell and await assistance.
The SF Fire Department will also help. The Coordinators should be advised of the people who need assistance.
B. Procedure:
All personnel should evacuate according to posted evacuation routes (see attachment) unless conditions make routes inaccessible, then the SFFD or UCPD will establish alternate evacuation routes.
When evacuating the AC-50, report to the corner of Arguello and Carl. (The red x on the map below) Be sure you report to your Emergency Coordinator or the person(s) in charge.
The staff will meet at the corner of Arguello & Carl.
VI. EMERGENCY CONDITIONS
A. FIRE
Procedure:
If you discover a fire
Call: 9-911 Give your name Your phone number The exact nature and location of the fire Other pertinent information
Pull the nearest manual fire alarm (outside the door, on the right side) Remove person(s) from affected area if immediately threatened. Close the door to the fire. Confine the fire by closing doors around smoke and fire in adjacent areas. After reporting the fire, you may attempt to fight the fire by using an extinguisher ONLY if:
You have been trained You think it is safe The fire is small and able to be contained
If you hear the fire alarm and evacuation is necessary
Follow the instructions given by the Fire Department, Police, or the designated Emergency Coordinator. Feel all doors for heat prior to opening them. If a door is hot, proceed to another exit. If smoke is present, stay low and crawl with body against the wall. The clearest air is near the floor. If forced to make a dash through smoke or flame, hold your breath and cover your nose and mouth. Use fire exit stairwells, not elevators, to exit the building. Do not return for personal belongings.
B. EARTHQUAKE PROCEDURES
During an Earthquake
IF YOU ARE INDOORS - Stay indoors. Take cover under a sturdy table or desk or brace yourself in a doorway or a corner. Move to an inside hallway when possible. Stay clear of windows, bookcases, cabinets, and other heavy objects. Watch out for falling plaster or objects. IF YOU ARE OUTDOORS Move to an open area away from power lines, power poles, trees, walls and chimneys. IF YOU ARE IN YOUR CAR o Pull to the side of the road and stop the car. o Do not park under bridges or overpasses or overhead wires.
Stay in your car until the shaking has stopped. Stay in your car if electrical wires have fallen across your vehicle. Do not attempt to cross bridges or overpasses that may have been damaged. IF YOU ARE IN THE ELEVATOR Do not panic. There is no danger. The elevators are equipped with an emergency call bell, an emergency telephone and a seismic switch which, if activated, automatically reduces the speed of the elevator and lowers it to the next stop. The doors will open and the elevator will shut down until the seismic switch is reset. When the elevator stops, exit the elevator and remain in the elevator lobby.
o o o
After the Earthquake
If able, report your status to your emergency coordinator. If you see or suspect: o Fire Activate the fire alarm and call 9-911. Close the door to the fire. o Gas leak Open windows/doors and leave the area. Call 9-911. Do not turn lights on or off, light matches, or do anything else that would cause a spark. o Electrical Hazard If you observe frayed wires, sparks or the smell of hot insulation, call 9911. o Water leak Contact the Facilities Coordinator immediately.
C. BOMB THREAT
In the event of a bomb threat, the person receiving the call should
Listen - do not interrupt caller. Do not risk breaking the phone connection by attempting to transfer the call to another party. Be observant of any identifying traits of the caller and note any background noises. Attempt to get the attention of another person and give him/her a note saying: "CALL 9-911 - BOMB THREAT." Ask the caller: o WHEN WILL IT GO OFF? o WHERE HAS IT BEEN PLACED? o WHAT DOES IT LOOK LIKE? o WHY ARE YOU DOING THIS? o WHO ARE YOU? o HOW WILL IT GO OFF? Forward all information to SFPD at 9-911. Notify the Department Emergency Coordinator.
D. TERRORIST ATTACKS (BIOLOGICAL WEAPONS)
Many of the precautions resemble those that we employed during the Unibomber threat in 1993. In particular, the campus asks everyone to exercise extreme caution in dealing with suspicious envelopes or packages, and to immediately call 9-911 if they encounter a suspicious envelope or package, or suffer what they believe may have been an exposure to infected material. The police will in turn consult appropriate experts as the circumstance demands. The following precautions were issued by Ara Tahmassian, assistant vice chancellorresearch services. Some characteristics of suspicious packages and letters include:
Handwritten or poorly typed addresses Incorrect titles or the use of a title, but no name Misspellings of common words Oily stains, discolorations or odor No return address Lopsided or uneven envelope Excessive security material such as masking tape, string, etc. Marked with restrictive endorsements, such as "Personal" or "Confidential" Shows a city or state in the postmark that does not match the return address
If you encounter an envelope with powder and powder spills out onto surface:
DO NOT try to CLEAN UP the powder. COVER the spilled contents immediately with anything (e.g., clothing, paper, trash can, etc.) and do not remove this cover. Then LEAVE the room and CLOSE the door, or section off the area to prevent others from entering (i.e., keep others away). · In order to avoid spread of contamination: Take your shoes off as you exit the work area, or cover them with a plastic bag to avoid tracking any substance around the building. Avoid touching door handles, ask someone to open the doors for you WASH your hands with soap and water to prevent spreading any powder to your face. Call 9-911 and Bio Safety Officer Brynte Johnson at 6-2097. REMOVE heavily contaminated clothing as soon as possible and place in a plastic bag, or some other container that can be sealed. This clothing bag should be given to the emergency responders for proper handling. SHOWER with soap and water as soon as possible. Do not use bleach or other disinfectant on your skin. If possible, list all people who were in the room or area, especially those who had actual contact with the powder. Give this list to Bio Safety Officer Johnson so that proper instructions can be given for medical follow-up, as well as to Campus Police for further investigation.
If you wish to use gloves when handling mail, they are available in most clinical areas and research laboratories. If you do not have access to any, please call the Office of Environmental Health and Safety (476-1300). Medical Center departments may call 3531837 at Parnassus, and 885-7536 at Mount Zion for gloves.
