My signature verifies that all drivers are properly insured, have a valid driver’s license, and will abide by all Sport Club/FGCU policies,
as well as local, state and federal laws. I understand that individuals who drive/rent a vehicle are personally responsible for any and
all damages (including property of a third party) that may occur as a result of driving said vehicle.
All FGCU affiliated travelers must be on an attached roster. Non-members may not travel with the Club, except coaches, staff
members and advisors. All students must have the 3 completed membership forms on file with the Sport Club Office at time of
submission. If any student does not have these on file, travel will not be approved.
