       include the executive). Include the following:

     ❏ Attach a list of all the planned activities for your organization for the upcoming year, this
       list should include the following information:
               ❏ Name of event
               ❏ Tentative date(s)
               ❏ Expected Attendance
               ❏ Location (be specific if known)
               ❏ Alcohol Present (Y/N) – If yes, is event licensed by your group?

The above information is for insurance purposes. It is very important that your group include all possible
functions. The Students’ Union will only extend its liability coverage to events that it has knowledge of
and has approved. Unplanned events may be granted coverage only upon the submission of a Campus
Group Supplemental Event Application Form to the USSU two (2) weeks prior to the event.
Note: Ratification can take up to two weeks to process once all information and fees have been
collected.
