REGISTRATION & PAYMENT POLICIES
Each form must be accompanied by the following: • $150 non-refundable REGISTRATION fee per student • $200 non-refundable DEPOSIT per student, which is applied toward preschool tuition balance • $200 PLACE-HOLDER fee per student, which is applied toward preschool tuition balance • Signed ARRANGEMENT FOR PAYMENT form • Preschool 2009-10 information sheet • A copy of student’s birth certificate for all new enrollees, including students entering the two-year-old program PLEASE NOTE: Registration fee, deposit and place-holder fee must be paid in full at the time the registration forms are submitted. The $200 place-holder fee will be refunded if child is withdrawn by March 31, 2009. SIBLING DISCOUNT: We are offering a 10 % sibling discount on the cost of the lower tuition. CANCELLATION & WITHDRAWALS: In accepting an enrollment, the school reserves a space for your child for the entire year. If for any reason enrollment must be cancelled, the school must be advised of the reason in writing 30 days prior to withdrawal or the parent/guardian will be responsible for the month’s tuition. If it is determined that the Zale Early Childhood Learning Center cannot accommodate the needs of your child, this contract may be terminated by the school and adjustment will be made. ARRANGEMENTS FOR PAYMENT: A signed “Arrangement for Payment” form, indicating your choice of payment, must accompany your registration form. After registration is completed, you will be notified of your financial commitment determined by your child’s class. FINANCIAL ASSISTANCE: Anyone wishing to apply for financial assistance must submit an application by March 13, 2009 to the Adolph and Rose Levis Jewish Community Center. Financial assistance applications are available after January 1, 2009 at the Zale School Office. All inquiries and applications are kept in strictest confidence. Those applying for financial assistance must pay the registration fee of $150 in full. In the event the financial assistance award is insufficient, the registration fee of $150 will be refunded or credited to your account. In this situation, please submit a refund request in writing to the school office immediately for processing. HEALTH & SAFETY: A State of Florida medical form #3040 and immunization form #680 must be submitted prior to the first day of school. These forms can be obtained at your doctor’s office. *No child will be permitted to attend the first day of school without these forms. ABSENCES & SCHOOL CLOSINGS: There will be no make-up days, refunds or credits in the event of absences, weather emergencies or other school closings.
