How to login:

Enter User ID and Password and click “Submit”.

NOTE: If this is the first time logging in and the Password is set to “password”,
enter the User ID and Password and click “Change Password”.

The Aggregate Victim Services Modules is divided into four sections.
Section A:
       Part 1: The percentage of total sexual violence funding by funding
               source.
       Part 2: Primary and Secondary Sexual Violence Victim Count
       Part 3: Demographics of New Primary and New Secondary Victims
       Part 4: Victim Information related to Law Enforcement and Human
               Trafficking

Section B: Unmet Needs

Section C: Disabilities

Section D: Services Provided

Section A
Part 1: The percentage of total sexual violence funding by funding source.

Enter the percentage of funding for each of the above funding sources.

Example 1:   Trust Fund                                     $22,000
             Block Grant                                    $30,000
             SASP                                           $12,000
             Other (United Way, Private Donations, VOCA)    $36,000
                                                            $100,000

Divide each of the funding sources by the Total Agency Victim Service Funding
($100,000) and indicate the percentages where applicable.




NOTES:       Percentages must total 100%. Enter “0” if not applicable.

If the agency is not going to report services in Part 7 that were provided by
Other funding sources (United Way, Private Donations, VOCA or other funding
sources the percentages would look like this:

Example 2:   Trust Fund                                     $22,000
             Block Grant                                    $30,000
             SASP                                           $12,000
                                                            $64,000

Divide each of the funding sources by the Total Agency Victim Service Funding
($164,000) and indicate the percentages where applicable. Do not include
services provided by other funding sources in Part 7.

Section A
Part 2: Primary and Secondary Sexual Violence Victim Count

Use the drop down box to indicate the Month and the free text box to indicate
the Year of which services were provided to victims.

Enter the Number of New Primary Victims. Include new victims in this count if
this is the first month they have received services.

Enter the Number of New Secondary Victims. Include new victims in this count if
this is the first month they have received services.

Enter the Number of Ongoing Primary Victims. Include victims in this count if
they are ongoing clients who have previously been reported as a New Primary
Victim.

Enter the Number of Ongoing Secondary Victims. Include victims in this count if
they are ongoing clients who have previously been reported as a New Secondary
Victim.

Section A
Part 3: Demographics of New Primary and New Secondary Victims
In this example, there were 7 new Primary victims and 6 new Secondary victims.
Therefore, each of the demographics collected must equal 13. If a demographic
for a victim is unknown, include this in the “Unknown” count.

Part 4: Victim Information related to Law Enforcement and Human Trafficking

Use this section to report on the number of primary victims who did not report to
Law Enforcement and the number of primary victims of human trafficking.

Click “Save” when finished. This completes Section 1.

Section B: Unmet Needs

Use the drop-down menu to select any unmet needs that apply to the victims
served during this reporting period. Enter the number of victims for each unmet
need and click “Save Unmet Need”.

After each unmet need is entered and saved it is added to a mini-report under
this section.




NOTE: If there is an unmet need that is not listed in the drop-down menu,
contact the SVDR Administrator to add the unmet need to the list.

Section C: Disability

Use the drop-down menu to select any disabilities that apply to the victims
served during this reporting period. Enter the number of victims for each
disability and click “Save Disability”.

After each disability is entered and saved it is added to a mini-report under this
section.




NOTE: If there is an unmet need that is not listed in the drop-down menu,
contact the SVDR Administrator to add the disability to the list.

Section D: Services Provided

Use the drop-down menu to select the service and enter a service count for both
primary and secondary victims. Click “Save Service”. Repeat this step until all
services have been entered.

Based on the percentages entered at the top of the page, the services are
allocated to each of the applicable funding sources and displayed in the mini-
report at the bottom of the page.
