This notice confirms the oral report I made to you when we met on [DATE]. At that
meeting I advised you that you have received a poor work performance rating for the

In order for you to correct the above problem(s), we agreed that it would be necessary for
you to adopt the following measures and steps:


I should like to affirm as strongly as possible that both our meeting and this
memorandum have one primary purpose: to improve your work performance to a
satisfactory level. We hope and trust that you will show improvement so that you can
remain an employee of the Company.

Please note that this warning memorandum has been sent to you in duplicate. Please sign
one copy after the words, “Receipt acknowledged,” and return that copy to me.
