Intent to Enroll - IU ACP C105 Chemistry/C125 Lab for 2011–12 Mishawaka High School Student name_____________________________________________________________________________ Current Grade 10 or 11 Current science class and teacher____________________________________ Advance College Project is a program administered by Indiana University which allows selected Indiana high schools to offer IU courses. Mishawaka High School’s Science Department offers C105 Principles of Chemistry I and C125 Experimental Chemistry. MHS students wishing to enroll in C105 and C125 must complete and submit this form to the counselor. Students will be notified if they are approved to enroll. Students who are approved by MHS may then follow the steps to take the course(s) for Indiana University credit through IU’s Advance College Project. See Mr. Gregg Smith or Mr. Dave Dobrzykowski for the IU course description. Student: Select one of the two options for this course: 1. ____College Credit—Approved students must pay Indiana University the current rate for five credit hours per year and earn at least a C (not C-) in each course. If the student plans to attend a school other than Indiana University, he/she has the responsibility to find out that school's rules for transfer of credits. The candidate must complete the IU ACP application, which is available from the MHS counselors. The IUSB Bursar’s Office will mail the tuition bill to the student at the home address. The student is responsible for meeting all of IUSB’s registration deadlines, including full payment of tuition. The tuition for this year-long course in 2009-10 was $433.25. Tuition has not been announced for 2011–12. Fee waivers are available for students who are participating in the National Free/Reduced Lunch program. Limited financial aid is also available through the IUSB ACP Office. Details will be available with the ACP applications in the spring of the 2010–11 school year.
