							It is a condition of proceeding with your application that you apply “enhanced”
CRB disclosure (There is a fee of £50 required to process CRB form).
Convictions and any criminal record information obtained through the Criminal
Record    Bureau’s   Disclosure   service   will   not   necessarily   be   a   bar   on
employment.

All circumstances will be taken into account: However any inconsistencies
compared with the information given above may invalidate your application.

At interview you will be required to bring with you the following information:-

      £50 Fee for CRB Form

      The application form if not already sent back to the office

      Name, address and contact details (Tel, fax, email) of two referees.

      CV if you have one prepared or full work history

      Current NMC PIN Card (if applicable/qualified nurses)

      Certificates relevant to practice or current training certificates e.g manual
       handling, first aid, fire etc. (PREP File if applicable) or NVQ certificates

      If not a British Citizen a letter of Indefinite Leave from the immigration
       office stated on your passport
Identification

For identification it is essential that you provide the following information to
process your application:-

      One form of group one identification and two forms of group one or
       two.      If you are unable to provide any of group one identification then
       you will need five documents from group 2.

      £50 pounds deposit towards your CRB check which in non refundable.
       You need to provide original documentation (No Photocopies) of the
       following group lists:-

Documents must be less than 3 months old.

All documents must be in your current name. At least ONE document must show your
current address and at least ONE document must show your date of birth.

Parking: Direct Nursing Services has its own car park situated at the back of the
building, unfortunately there is limited space,

Directions: turn down Picton Road the road located by Sweet Dream bed shop at the corner
then turn right go pass the garage and the first wooden gate is Direct Nursing Services.

Or alternatively there is a free car park at Skewen train station on Station Road

Directions: as you come out of the train station car park walk to the end of the road and turn
left you will see Cresci café on your left hand side and walk down the road towards Direct
Nursing Services.




The above information that has been requested by DNS is essential to your application. If it is
brought to interview it will speed up your application and it can then be processed immediately
after your interview.
