REASONS FOR EXECUTIVE TRACK

        Application for Charter has not been signed by Church Council Members or 30 Adult Members of the Congregation

George Jones has been pastor of the Waukegan Church for more than 22 years. Throughout all of this time the church has been
operated as if it were in fact a Charter Church, based on the belief of the Pastor, the Church Council, and the congregational members
throughout the period that the church previously had received a Charter. The Pastor believes that the church must have been chartered
in 1942 when the church first bought property, but that all subsequent records of a previously granted Charter have been lost. He
states that asking the Church Council and the membership to sign a Charter application at this time – after the church has been part of
the Foursquare family for 78 years - could invite criticism of, and a mistrustful attitude towards, the national church leadership. As
such, he is requesting, with the concurring recommendation of Supervisor Dan Mundt, that the church be granted a retroactive Charter
– to be dated January 1, 1942 (the year the church first bought property) – without a fully signed Charter application.

Church Charter based on organized church and Supervisor Recommendation.

RESOLVED: By the Executive Staff Committee of the Board of Directors of International Church of the Foursquare Gospel, a
religious corporation, that upon the recommendation of the Heartland District Supervisor, Church Charter be granted the
WAUKEGAN, ILLINOIS #32129 Foursquare Church, with such Charter to be effective January 1, 1942, on the basis of the
church’s status as an organized church for the past 78 years.

REASON FOR EXECUTIVE TRACK:
- Lease payment exceeds 30% of income (154% of historical income, 58% of projected income)

There is a 60-day out clause. The District has guaranteed the lease payments. The church plans to pay for this lease through an
increase in monthly church planting funds and by sharing the facility with two local Foursquare churches.

Requesting permission to lease approximately 1,541 square feet located at 1014 East Grand Street to be used as a church facility for a
period of 12 months beginning September 1, 2009 and ending August 31, 2010 for $1,200 per month.

They project their income will increase from $781 per month ($281 tithes and offerings + $500 church planting funds) to $2,081 per
month ($281 tithes and offerings + $1,000 church planting funds + $800 rent-sharing). The lease payment would then be 58% of their
income.

The church opened in March 2009. They received $500 per month church planting funds from March 2009 to August 2009. They will
receive $1,000 per month from September 2009 through February 2011.
They plan to share the facility with two other Foursquare churches. Elizabeth Uptown, NJ #32769 will contribute $300 per month and
a Bible study from Roselle, NJ #31693 will contribute $500 per month. The ARC has received written confirmations from both parties
of their intent to share the space.

approximately 1,541 square feet located at 1014 East Grand Street, to be used as a church for a period of one (1) year beginning
September 1, 2009 and ending August 31, 2010 for One Thousand Two Hundred Dollars ($1,200) per month.

The loan with First Tennessee Bank came due on May 28, 2009. The church has been unable to find a new loan from another lender
and so requested an extension from First Tennessee Bank. The bank agreed to extend the loan until February 28, 2010 (nine months)
to give the church more time to find a new loan. The bank has just provided the documents to extend the loan maturity date.

REASONS FOR EXECUTIVE TRACK:
- Outside loan over $500,000
- Down payment of 7.7% instead of standard 20%.
- Contingent on satisfactory appraisal. Bank has ordered new appraisal but it is not yet completed. Last appraisal was done September
18, 2008.
- Requesting to use bank’s environmental screen report as substitute for standard EDR report. The bank will order the report once
Foursquare has signed the loan commitment letter. Board approval will be contingent on satisfactory review of the screen report.

The purchase price is $650,000, and the church has a down payment of $50,000 (7.7%). Requesting approval based on 67% loan-to-
value. LTV is based on 9/18/08 appraisal the value is $898,000, giving them an initial loan to value ratio of 67%.

A new appraisal has been ordered but is not yet completed. The church is asking for approval prior to its completion because the bank
needs Foursquare to sign the loan commitment to initiate its environmental due diligence and title work, which should be done this
week to try to meet closing (scheduled for September 24). If the new appraised value is more than $858,000 (70% LTV) the terms of
the loan will not change.

The bank requires its own environmental screen report. The church is required to pay for the bank’s report, so they are requesting to
use the bank’s screen report in lieu of the standard EDR report to avoid paying for duplicate reports. The bank will not order the
report until Foursquare signs the commitment letter. The Board’s approval will be contingent on a satisfactory review of the bank’s
environmental screen report.

PROJECT DESCRIPTION: To purchase property comprised of a sanctuary and two residential buildings, to be used as a church
facility and offices, and to obtain a loan from John Marshall Bank.

The Mid-Atlantic District has contributed $10,000 toward the purchase of this property.

ENVIRONMENTAL:
LAND: The bank requires its own environmental screen report. Since the church is required to pay for the bank’s screen report they
are requesting the Board’s approval without first obtaining the standard EDR report, to avoid paying for two similar reports. The
Board’s approval will be contingent on a satisfactory review of the bank’s environmental screen report by the ARC.

STRUCTURE EXISTS:

  TESTING DONE: Asbestos, lead paint, PCBs, radon.
    RESULTS: Asbestos – 4,300 sq. ft. of floor tiles in the church and 900 sq. ft. of floor tiles in the Fairfax Blvd. house contain
asbestos. No asbestos was identified in the 6th Avenue house.
                 Lead paint – No lead-based paint identified in the church. In the Fairfax Blvd. house less than 50 sq. ft. of flooring by
the basement door are painted with a lead-based paint. In the 6th Avenue house approximately 1,650 sq. ft. of floors are painted with a
lead-based paint.
                 PCBs – None
                 Radon – 3.0pCi/L, less than the EPA’s recommended action level
    EST. ABATEMENT COSTS: $16,700 = $14,200 to remove all asbestos flooring + $2,500 to remove all lead-based paint.
    CHURCH PLAN: The seller has agreed to pay for half of the asbestos abatement costs, leaving the church to pay $9,600, which
they have in savings.

maximum price of Six Hundred Fifty Thousand Dollars ($650,000), said property to be vested in the name of INTERNATIONAL
CHURCH OF THE FOURSQUARE GOSPEL; and that approval be granted to obtain a loan from John Marshall Bank for Six
Hundred Thousand Dollars ($600,000) at 6.5% interest for 25 years, due in 3 years, said loan to be secured by a mortgage or deed of

based on the following information that was provided from Pastor Mark Hladek, divisional superintendent for the Utah area. Pastor
Mark felt that the following information would be helpful for the Board and is regionally significant:

   1. Delta is both a rural and remote location with a fairly small population, something around 3,000.

   2. Our churches in that region face similar challenges with similar result, though Georgeann has had better success and has greater
   promise in my opinion.

   3. Challenges faced by churches in this region are not just geographical, but spiritual as we have found that the further one is from
   population centers the stronger the strongholds of Mormonism. Of course, every area has its spiritual powers and principalities to
   be dealt with, but we almost universally recognize that Mormonism is a spiritual force to be reckoned with mightily, tenaciously
   and strategically. The percentage of Mormon Church members goes up dramatically as you move out from more populated areas.
   For example, along the Wasatch front in Utah, a stretch of the most populated area in the state from Provo to Brigham City (80%
   of the population in an 80 mile stretch along the I-15), the percentage of non-Mormons may be approximately 40%, except the
   Provo area. Moving out to more distant locations it drops dramatically to as low as 1-5%. All this said to just note the challenges
   these faithful pastors face which are often reflected in their reports.

   4. Georgeann is a woman, and women in rural and remote communities in Utah and the greater region, which Brigham Young
   called the "Territory of Deseret," have the added challenge of overcoming "a woman's place" which is not, in that culture, a place
   of leadership. Georgeann has had to face this squarely over the years and done so well. That spirit and attitude is not just present in
   the Mormon population, but in the population as a whole as subtle and not so subtle spiritual influence.

Ordination based on Rule A, holders of International License

District Supervisor Ron Pinkston, along with the approval of Regional Supervisor, Robby Booth, is requesting International License
and Ordination concurrently based on the following:

Pastor Dillard has been a licensed minister with the Foursquare organization for over 20 years and received her District license in
March 1998. Georgeann has an educational opportunity that will open up further influence in the Delta community. An Ordination is
required for that door to be opened. Based on the above, the request is being made for Ordination. Pastor Ron also spoke with Mark
Hladek, Georgann’s superintendent in Utah and he gives her his highest recommendation noting that in his opinion and contacts with
her, not securing an International license and then progressing to Ordination has not been out of any lack of responsibility, but rather a
focus on life and ministry at hand without preoccupation with self-promotion. Pastor Mark also shared the following special
circumstances she has and is navigating.

   1. Georgann was intimately and actively involved with her husband as they planted the Delta Foursquare Church and became the
      pastor shortly after her husband’s sudden death, carrying the responsibilities of that young church admirably while at the same
      time dealing with their family business.

   2. She then tragically lost her teen daughter in an automobile accident on an icy morning as the daughter drove herself to school.
      She has come through that loss well and continued to love her church and community.

   3. Georgann has been and continues to grow in influence in her community, but is limited in that rural Mormon culture both
      because she is not Mormon and is a Christian woman/pastor. The term “Ordained” and the faith and credibility it affords from
      our International church is significant. Ordination will open doors of opportunity that Georgann will most certainly prove
      faithful to walk through.

    4. As a Foursquare pastor she has been consistently faithful and supportive and an engaged part of the Intermountain division.
        She understands our family values and is committed to living them out.
