           GENERAL SURVEY INFORMATION

           As a survey participant, you will receive a hardcopy survey report book accompanied by two CDs. The hardcopy
           report contains Sections I – VI of the survey (formerly Volume 1). The first CD contains Section VII of the survey
           (formerly Volume 2) in a PDF format. Section VII includes total cash compensation and productivity data
           reported nationally, regionally and by organization type. The second CD contains detailed electronic data tables
           in Excel format. An invoice for $500 (plus $21 shipping and handling) will be sent with the report at that time. You
           may also elect to purchase a printed hardcopy version of Section VII for an additional cost of $150.

           GENERAL SURVEY INSTRUCTIONS

           Please complete one survey for EACH reporting entity within your organization. For example, if you have three
           hospitals, a separate survey should be completed for each hospital.
           Please report data as of February 1, 2011.

           Return the completed survey questionnaire by May 2, 2011. When you have completed this questionnaire,
           email to surveysubmission@sullivancotter.com.

           Sections One and Three of the survey MUST be completed in order for your organization to be
           considered a participant.

           Some questions in Sections One and Two have follow-up questions which appear as “pop-ups” based on your
           answer to a given question. For example, Question 3 in Section One asks if your organization is for-profit or not-
           for-profit. If you answer for profit, a pop-up question appears asking if your organization is publicly traded. From
           the Main Menu, you are able to access survey definitions, a complete list of specialties surveyed, and a printable
           version of this questionnaire in PDF format. It is recommended that you review and/or print these reference
           documents before completing the questionnaire.
           Section One

           Section One requests general information related to the organization including tax status, organization type, and
           key organizational scope metrics. Section One also requests information regarding the increase or decrease in
           provider counts within your organization. Reminder: Section One must be completed in order to be
           considered a participant.

           Section Three requests comprehensive information for seven major provider areas:

              Clinically-oriented Physicians (Staff Physicians and Program Directors)

              Mid-Level Providers
              PhDs
              Medical Group Executives


           Data for each of the seven provider areas are collected on separate, labeled tabs within the questionnaire. The
           definitions below reflect all the data elements this survey collects. However, requested data elements vary by
           provider area. Therefore, it is recommended that you print these definitions to use as a reference while
           completing the questionnaire. These definitions are also available on the “Definitions” tab in the questionnaire.

           If you have physician compensation data in a computerized database (e.g., Excel spreadsheet), it may be
           possible to submit it in lieu of completing Section Three. Also, if you participated in the 2010 survey and would

           DATA DEFINITIONS FOR COMPLETING SECTION THREE:
           CASH COMPENSATION AND PRODUCTIVITY

           Please pro-rate compensation and productivity data to reflect a full calendar year for incumbents who have been
           employed for less than a full calendar year.

           Column (1) Your Organization’s Internal Tracking ID. List each physician, PhD or mid-level provider individually by title,
           name, employee number or other internal identification. This information is used to help you track the data that you report.
           This information is not used by SullivanCotter for any other purpose and is not included in the survey report.

           Column (2) YSR (Years Since Residency). Report the number of years since the incumbent completed his/her residency.
           For PhDs and mid-level providers, record the number of years since the incumbent completed school.

           Column (3) Total FTE (Full-Time Equivalent). Record the total full-time equivalent status of the incumbent that
           corresponds with time spent performing all duties. For example, if the incumbent works full-time, record 1.0; if the
           incumbent works 75% of the time, record .75. This information must be completed in order for your organization's
           data to be included in the cash compensation statistics.

           Column (4) Clinical FTE. Report the full-time equivalent status of the incumbent that corresponds with time spent
           performing clinical duties only, including time spent in direct and indirect patient care. For example, if the incumbent works
           full-time, and 100% of the incumbent’s work effort is spent in direct or indirect patient care, record 1.0; if the incumbent
           works full time, but only 75% of the work effort is spent in direct or indirect patient care, record 0.75. This information is
           desired; however, it is not required.


           Column (5) Administrative, Research, and Teaching (ART) FTE. Report the full-time equivalent status of the incumbent
           that corresponds with time spent performing ART duties only. For example, if the incumbent works full-time, and 25% of the
           incumbent’s work effort is spent performing ART duties, record 0.25. This information is desired; however, it is not required.

           Column (6) Clinical Base Salary. Report the actual annual base salary paid to each incumbent for time spent performing
           clinical duties, including time spent in direct and indirect patient care as of February 1, 2011. Report this information in
           thousands (e.g., $140,000 becomes $140.0).

           Column (7) ART Base Salary/Stipend. Report the actual annual base salary/stipend paid to each incumbent for time
           spent performing ART duties as of February 1, 2011. Report this information in thousands (e.g., $140,000 becomes
           $140.0).

           Column (8) Department Chairs, Medical Group Executives and Mid-Level Providers Only - Total Base Salary.
           Report the actual annual base salary paid to each incumbent as of February 1, 2011. Report this information in thousands
           (e.g., $140,000 becomes $140.0).

           Column (9) Total Incentive Payment. Report the total incentive payment for the most recently completed fiscal year.
           Incentive compensation is typically based on quality or productivity measures and thus varies with annual performance . If
           your organization pays incentives more than once per year, report the total annual incentive compensation paid. Report this
           information in thousands (e.g., $10,000 becomes $10.0).

           Column (10) % Non-productivity Based. Report the percentage of the total incentive payment that is attributed to non-
           productivity based measures.

           Column (11) Other Cash Compensation. Report any other cash compensation paid to the physician in the most recently
           completed fiscal year, such as honoraria, longevity bonuses, retention bonuses, profit-sharing, sign-on bonuses, long-term
           incentive payments and the like. Report this information in thousands (e.g., $1,500 becomes $1.5). Do not include on-call
           pay or pay for extra work such as moonlighting.

           Column (12) Qualified Deferred Compensation. Report any employer contributions to qualified defined benefit or
           contribution plans (e.g., 401(k), 403(b)) for the most recently completed fiscal year. Do not report contributions made by the
           incumbent. Report this information in thousands (e.g., $12,500 becomes $12.5).

           Column (13) Non-Qualified Deferred Compensation. Report any employer contributions to non-qualified retirement
           plans for the most recently completed fiscal year. Do not report contributions made by the incumbent. Report this
           information in thousands (e.g., $12,500 becomes $12.5).

           Column (14) Continuing Medical Education Expenses. Report the annual amount of employer costs for continuing
           medical education expenses. Report this information in thousands (e.g., $2,500 becomes $2.5).

           Column (15) Total Cost of Benefits. Report the annual employer benefit cost for employed providers. This includes the
           cost of health, life and disability insurances; employer contributions to qualified defined benefit and contribution plans (e.g.,
           401(k), 403(b)) and non-qualified retirement plans; continuing medical education (CME) expenses; FICA, payroll and
           unemployment taxes; and professional license fees. Do not include the cost of malpractice insurance or paid time off.
           Report this information in thousands (e.g., $30,000 becomes $30.0).

           Column (16) Work RVUs. Report the number of work Relative Value Units (wRVUs) performed by the incumbent during
           the most recently completed fiscal year. Included in this calculation are: wRVUs using the Resource Based Relative Value
           Unit Scale published by the Centers for Medicare and Medicaid Services (CMS) for all payors. The wRVUs should be
           based on work personally performed by the incumbent and should include any adjustments made by modifier usage. Do
           not include wRVUs attributed to: physician extenders; the technical component for laboratory, radiology or other
           procedures not personally performed by the physician; or wRVUs that have been weighted by a conversion factor.
           Do not include the practice expense RVU (peRVU) or the malpractice expense RVU (mRVU). This information is not
           requested in thousands. For example, 3,450 wRVUs should be reported as such.
           Note: Please indicate the CMS Physician Fee Schedule Year that corresponds with the reported wRVUs using the drop-
           down menu in the wRVU column header.

           Note: When reporting for mid-level providers, report data reflecting wRVUs personally performed by the mid-level provider.
           Note: For Anesthesiologists and Certified Registered Nurse Anesthetists, instead of wRVUs, report American Society of
           Anesthesiologists (ASA) units. Include all components of ASA units (base units, time in 15 minute increments and risk
           factors).

           Column (17) Collections. Report the collections generated for all direct professional services provided by the incumbent
           during the most recently completed fiscal year. Include collections for: fee-for-service payments; capitation payments
           allocated to the physician; and payments for the administration of immunizations and chemotherapy drugs. Do not include
           collections for: services provided by physician extenders; the technical component of laboratory, radiology,
           medical diagnostic or surgical procedures; collections related to infusion or drug charges; or any collections
           associated with retail income (e.g., optical, pharmacy, hearing aids, etc.). Report this information in thousands (e.g.,
           $250,000 becomes $250.0).
           Note: When reporting for mid-level providers, report data reflecting collections generated from services personally
           performed by the mid-level provider.


           Column (18) Gross Patient Charges. Report the gross patient charges generated for all direct professional services
           provided by the incumbent during the most recently completed fiscal year. Include: fee-for-service charges; capitation
           charges allocated to the physician; and charges for the administration of immunizations and chemotherapy drugs. These
           should not be adjusted for such deductions as bad debt, contractual allowances, professional courtesy, etc. Do not include
           charges for: services provided by physician extenders; the technical component of laboratory, radiology, medical
           diagnostic or surgical procedures; infusion or drug-related charges; or any charges associated with retail income
           (e.g., optical, pharmacy, hearing aids, etc.). Report this information in thousands (e.g., $450,000 becomes $450.0).

           Note: When reporting for mid-level providers, report data reflecting charges generated from services personally performed
           by the mid-level provider.


           Column (19) Medical Directors/Division Chiefs Only - Total Service Area Operating Expenses. Report all expenses
           associated with operating the service area incurred during the most recently completed fiscal year. Include salaries,
           employee benefits, purchased services, supplies, professional fees, depreciation, rentals, interest, and insurance. Exclude
           bad debts or income taxes. Report this information in millions (e.g., $3,250,000 becomes $3.25).

           Column (20) Medical Directors/Division Chiefs and Department Chairs Only - Number of Physicians Responsible
           for. Report only the total number of physician FTEs for which the incumbent is directly responsible.

           Column (21) Medical Directors/Division Chiefs Only - Number of Sites Responsible for. Report the number of
           sites/locations for which the incumbent is directly responsible.

           Column (22) Department Chairs Only - Total Department Operating Expenses. Report all expenses associated with
           operating the department incurred during the most recently completed fiscal year. Include salaries, employee benefits,
           purchased services, supplies, professional fees, depreciation, rentals, interest, and insurance. Exclude bad debts or income
           taxes. Report this information in millions (e.g., $3,250,000 becomes $3.25).

           Column (23) Department Chairs Only - Department Net Operating Revenue. Report the net operating revenue for the
           department collected during the most recently completed fiscal year. Net operating revenue is defined as gross revenue
           less deductions for contractual arrangements, discounts, charity care, and bad debt, etc. Include all sources of revenue,
           including research/grant funding. Report this information in millions (e.g., $3,250,000 becomes $3.25).

           Column (24) Department Chairs Only - Number of Residents and/or Fellows in Department. Report only the total
           number of resident and/or fellow FTEs in the department for which the incumbent is directly responsible.

           Column (25) Administrative MDs Only - Number of Physicians with Medical Staff Privileges. Report the total number
           of physician FTEs which provide health care within the hospital and/or health system. Include both employed physicians
           and contractual physicians.

           Column (26) Administrative MDs Only - Total Center/Service Line Operating Expenses. Report all expenses
           associated with operating the Center or Service Line incurred during the most recently completed fiscal year. Include
           salaries, employee benefits, purchased services, supplies, professional fees, depreciation, rentals, interest, and insurance.
           Exclude bad debts or income taxes. Report this information in millions (e.g., $3,250,000 becomes $3.25).

           Column (27) Administrative MDs Only - Center/Service Line Net Operating Revenue. Report the net operating
           revenue for the Center or Service Line collected during the most recently completed fiscal year. Net operating revenue is
           defined as gross revenue less deductions for contractual arrangements, discounts, charity care, and bad debt, etc. Report
           this information in millions (e.g., $3,250,000 becomes $3.25).

           Column (28) Medical Group Executives Only - Total FTEs in Medical Group. Report the total number of FTEs in the
           medical group, including physician and non-physician FTEs.

           Column (29) Medical Group Executives Only - Total Physician FTEs in Medical Group. Report only the total number
           of physician FTEs in the medical group.

           Column (30) Medical Group Executives Only - Medical Group Net Operating Revenue. Report the net operating
           revenue for the medical group collected during the most recently completed fiscal year. Net operating revenue is defined as
           gross revenue less deductions for contractual arrangements, discounts, charity care and bad debt, etc. Report this

           PHYSICIAN POSITION LEVELS

           There are five physician position levels included in the survey:

           * Staff Physician
           * Program Director
           * Medical/Program Director: Resident Education
           * Medical Director/Division Chief
           * Department Chair

           Each of the physician position levels is described below.

           Staff Physician
           Provides medical care to patients the majority of time. May engage in teaching and research and/or may have
           responsibility for residents. Typically devotes at least 75 percent of work time to direct or indirect patient care.

           Program Director
           Responsible for managing and directing the services for a specific program within a division. Examples include: Program
           Director, Vascular Lab; Program Director, Sleep Center; and Program Director, Cardiac Cath Lab. Typically devotes 10-25
           percent of work time to administrative duties related to program management.

           Medical/Program Director: Resident Education (for 15 major clinical specialty areas only)
           Responsible for managing and directing the resident education program within a specialty practice area. This position has
           responsibility for curriculum planning, implementation, and monitoring. Ensures the quality of the teaching faculty through
           the development of educational programs and the process of assessment and feedback. Assumes primary responsibility
           for resident recruitment and provides mentorship and supervision to residents. Typically devotes 20-50 percent of work
           time to administrative duties related to resident education.

           Medical Director/Division Chief
           Responsible for managing and directing the medical services for a specific program or division. This position is generally
           the second level physician manager position within a large department. For example, the Medical Director/Division Chief of
           Gastroenterology would report to the Department Chair of Medicine. Typically devotes 20-50 percent of work time to
           administrative duties related to service area management.

           Department Chair
           Responsible for the financial and operating results achieved by the department. This position is the top physician
           management position for a major medical department within the organization. Typically devotes 90-100 percent of work
           time to administrative duties related to department operation.


           PhD POSITION LEVELS

           There are three PhD position levels included in the survey:

           * PhD - Staff Clinician/Researcher
           * PhD - Principal Investigator
           * PhD - Department Chair

           Each of the PhD position levels is described below.

           PhD – Staff Clinician/Researcher
           Responsible for providing clinical services and/or conducting research in area of expertise. May provide patient care and
           diagnoses of patients’ conditions. May develop and monitor adherence to research protocols, conduct and/or review
           various phases of research projects and ensure timeframes are met. May prepare or assist in the preparation of grant
           proposals. Provides expert advice to a variety of professionals and health care providers, including physicians.

           PhD – Principal Investigator
           Responsible for leading research projects and ensuring proper protocols are followed. Manages all technical, financial,
           compliance and administrative components of the projects. Directly responsible for submission of grant proposals, and
           completion of funded projects. Ensures that projects are carried out in compliance with the terms, conditions, and policies
           of the funding agency.


           PhD – Department Chair
           Responsible for the financial and operating results achieved by the department. This position is the top PhD management
           position for a major clinical research department within the organization. Promotes curriculum development and
           educational innovation within the department, and fosters collaboration with other departments.


           PHYSICIAN ADMINISTRATIVE POSITIONS

           There are 30 physician administrative positions in the survey, each of which is described below.

           SYSTEM/HOSPITAL LEVEL ADMINISTRATIVE MEDICAL DIRECTORS

           Chief Medical Officer/Vice President, Medical Affairs (CMO/VPMA)
           This position is appointed by the Board and/or CEO and is responsible for overall direction of the medical staff. As a
           member of the organization's senior management team, participates in the development of short- and long-term plans and
           strategies for the organization. Generally does not practice medicine. This position requires a licensed physician.

           Chief of Staff - Elected
           This position is elected by the medical staff and acts as a liaison between the medical staff and administration. Generally
           practices medicine for at least 50 percent of work time. This is typically a one to two-year term and may or may not be
           compensated by the organization. This position requires a licensed physician.

           Medical Director - Bioethics
           Chairs the Bioethics Committee and oversees development of policies as they relate to patient rights and Bioethics.
           Provides direction and guidance on critical and complex ethical issues. This position requires a licensed physician.

           Medical Director - Clinical Transformation
           Responsible for the comprehensive, ongoing approach to ensure care delivery excellence that measurably improves
           quality, enhances service, and reduces costs through the effective alignment of people, processes, and technology. This
           position requires a licensed physician.

           Medical Director - Credentialing

           Chairs the Credentials Committee and oversees the development of policies and procedures related to the same. Acts as
           a liaison between the medical staff and the credentialing office staff to ensure compliance with protocols and efficiency with
           regard to medical staff appointments. Generally this position is not a full-time responsibility. This position requires a
           licensed physician.

           Medical Director - Education
           Responsible for overall direction of medical education within the organization. Ensures all education programs, including
           resident education, comply with protocols. Oversees development of the organization's medical education curriculum.
           This position requires a licensed physician.

           Medical Director - Information Technology
           Responsible for overseeing and directing the physician medical Information Technology (IT) program. Ensures physician
           IT needs are met. This position requires a licensed physician.

           Medical Director - Minimally Invasive Surgery/Robotics
           Responsible for overseeing and directing the medical activities associated with the Minimally Invasive Surgery/Robotics
           Program. Responsible for ensuring the Minimally Invasive Surgery/Robotics Program is in compliance with medical quality
           standards. This position requires a licensed physician.

           Medical Director - Patient Safety
           Responsible for the primary oversight of organization or system-wide patient safety initiatives. Directs process
           improvements that will support the reduction of patient harm. Provides leadership to both clinical and non-clinical
           resources for quality improvement and patient safety programs. The position requires a licensed physician.

           Medical Director - Physician Integration
           Responsible for supporting and forming positive cultural, operational and clinical efficiencies between a network of
           physicians and a health system or hospital. Responsible for identifying products, services, technologies and other
           relationship needs of the physician community as well as the health system/hospital. Acts as a liaison to ensure effective
           relationships are continually fostered. This position requires a licensed physician.

           Medical Director - Quality Assurance
           Responsible for developing and coordinating programs designed to ensure medical practices meet the organization's and
           third party quality goals and objectives. This position requires a licensed physician.

           Medical Director - Research
           Responsible for planning and overseeing the research activities throughout the organization. Works with a variety of
           physicians and PhDs to ensure that research studies follow appropriate protocols, are on schedule, and are meeting the
           objectives of the study. Participates in the preparation of grant proposals and acts as a liaison between funding agencies
           and the organization. This position requires a licensed physician.


           Medical Director – Simulation Center
           Responsible for managing and directing the education and research functions of the Simulation Center. Ensures that all
           programs comply with protocols. Oversees the development of instructional curriculum of the Simulation Center. This
           position requires a licensed physician.

           Medical Director – Translational Medicine
           Responsible for coordinating the pursuit and validation of innovative clinical developments that bridge many disciplines to
           develop the highest quality care that meets the organization's safety and efficacy standards. Oversees interdisciplinary
           drug and clinical trials, NIH and other grant projects. Effectuates the process between clinical discovery and full clinical
           implementation. This position requires a licensed physician.

           Medical Director – Trauma
           Responsible for managing the multidisciplinary activities related to the delivery of acute trauma care. Responsible for
           managing the quality of care, volume, scope of services and cost-effectiveness of the trauma service. Organizes and
           manages the overall physician/surgeon component of the trauma service. This position requires a licensed physician.


           Medical Director - Utilization Review
           Responsible for managing and directing the organization's Utilization Review program designed to oversee the
           appropriateness and necessity of medical care provided. This position requires a licensed physician.


           Medical Director - Wound Care
           Responsible for managing and directing the organization’s Wound Care Program. Responsible for ensuring the Wound
           Care Program meets quality, operating and financial objectives. This position requires a licensed physician.

           CENTER/SERVICE LINE LEVEL ADMINISTRATIVE MEDICAL DIRECTORS

           Medical Director - Cancer Center
           Responsible for managing and directing activities of a Cancer Center. Administers Cancer Center programs and policies.
           Responsible for ensuring the Cancer Center meets quality, financial and operating objectives. This position requires a
           licensed physician.

           Medical Director - Dialysis Center
           Responsible for managing and directing activities of a Dialysis Center. Administers Dialysis Center programs and policies.
           Responsible for ensuring the Dialysis Center meets quality, financial and operating objectives. This position requires a
           licensed physician.

           Medical Director - Hospice
           Responsible for managing and directing the medical activities of the organization’s Hospice program. Responsible for
           ensuring the Hospice program is in compliance with medical quality standards. This position requires a licensed physician.


           Medical Director - Long-Term Care
           Responsible for managing and directing the medical activities of the organization’s Long-Term Care Program. Responsible
           for ensuring the Long-Term Care Program is in compliance with medical quality standards. This position requires a
           licensed physician.

           Medical Director - Multi-Specialty Group
           Responsible for managing and directing the activities of a Multi-Specialty Group (generally three or more specialties) and
           administering the group's programs and policies. Ensures clinic operations meet financial, quality and productivity goals.

           MEDICAL GROUP EXECUTIVE POSITIONS

           There are eight medical group executive positions.

           CEO - Medical Group, MD
           Responsible for establishing and achieving the medical group's short- and long-term objectives and overall viability.
           Reports to the Board of the medical group. This position requires a licensed physician.

           CEO - Medical Group, Non-MD
           Responsible for establishing and achieving the medical group's short- and long-term objectives and overall viability.
           Reports to the Board of the medical group. This is a non-MD position.

           Chief Operating Officer
           Responsible for overseeing and coordinating the daily operations of the medical group. Works closely with managers and
           supervisors to ensure daily activities are completed in accordance with established goals, objectives and protocols.
           Identifies areas in need of improvement and recommends operational changes. Reports to the medical group’s CEO. This
           position does not require a licensed physician.


           Chief Compliance Officer
           Responsible for planning, overseeing and directing programs to ensure the medical group is in compliance with state and
           federal laws and regulations. Reports to the CEO or CFO of the medical group.

           Chief Financial Officer
           Responsible for managing finance, treasury and accounting operations of the medical group. Works to establish financial
           plans and forecasts for the organization based on health care economics. May have responsibility for Information
           Systems. Reports to the medical group’s CEO.

           Chief Human Resources Officer
           Responsible for all human resources management and development programs and procedures, including employment,
           compensation and benefits, employee/labor relations, education and training, employee health and safety, and compliance
           with employment laws and regulations. Reports to the medical group’s CEO.

           Chief Information Officer
           Responsible for planning, overseeing and directing the provision of information services, including mainframe and network
           operations, system development and operations, applications development and programming, and telecommunications.
           Reports to the CEO, COO or the CFO of the medical group.

           Chief Legal Counsel
           Responsible for planning, managing and coordinating the legal affairs of the medical group. Directs and coordinates
           activities of outside counsel. Responsible for ensuring organizational activities meet legal and regulatory requirements.
           Provides legal guidance with the goal of reducing risk to the medical group. This position requires a licensed attorney.

           ADMINISTRATIVE PhD POSITIONS

           There are two administrative PhD positions included in this survey. Each is described below.

           Director of Academic Affairs (PhD)
           Responsible for ensuring residency and fellowship programs meet accrediting agency requirements. Meets with third party
           reviewers to review and explain educational programs within the organization. Participates in the development of
           educational programs and curriculum. Coordinates the maintenance of records regarding the educational activities and
           accomplishments of each physician.


           Director of Research (PhD)
           Responsible for planning and overseeing the research activities throughout the organization. Works with a variety of
           physicians and PhDs to ensure that research studies follow appropriate protocols, are on schedule, and are meeting the
           objectives of the study. Participates in the preparation of grant proposals and acts as a liaison between funding agencies
           and the organization.



           DATA DEFINITIONS FOR COMPLETING SECTION THREE:
           CASH COMPENSATION AND PRODUCTIVITY

           Please pro-rate compensation and productivity data to reflect a full calendar year for incumbents who have been

           Column (2) YSR (Years Since Residency). Report the number of years since the incumbent completed his/her
           residency. For PhDs and mid-level providers, record the number of years since the incumbent completed school.

           Column (3) Total FTE (Full-Time Equivalent). Record the total full-time equivalent status of the incumbent that
           corresponds with time spent performing all duties. For example, if the incumbent works full-time, record 1.0; if the
           incumbent works 75% of the time, record .75. This information must be completed in order for your organization's
           data to be included in the cash compensation statistics.

           Column (4) Clinical FTE. Report the full-time equivalent status of the incumbent that corresponds with time spent
           performing clinical duties only, including time spent in direct and indirect patient care. For example, if the incumbent works
           full-time, and 100% of the incumbent’s work effort is spent in direct or indirect patient care, record 1.0; if the incumbent
           works full time, but only 75% of the work effort is spent in direct or indirect patient care, record 0.75. This information is

           Column (5) Administrative, Research, and Teaching (ART) FTE. Report the full-time equivalent status of the
           incumbent that corresponds with time spent performing ART duties only. For example, if the incumbent works full-time,
           and 25% of the incumbent’s work effort is spent performing ART duties, record 0.25. This information is desired; however,
           it is not required.

           Column (6) Clinical Base Salary. Report the actual annual base salary paid to each incumbent for time spent performing

           incentive payments and the like. Report this information in thousands (e.g., $1,500 becomes $1.5). Do not include on-

           Column (12) Qualified Deferred Compensation. Report any employer contributions to qualified defined benefit or
           contribution plans (e.g., 401(k), 403(b)) for the most recently completed fiscal year. Do not report contributions made by

          Call Coverage Details: Please indicate whether your hospital provides call coverage for a particular service based on the type of call. Restricted call arrangements require the physician to remain on premises, while unrestricted
          call arrangements require the physician to present to the hospital within a specified time frame. Indicate if the call is for trauma coverage, the number of physicians sharing in providing call coverage for the service, and whether
          employed physicians share in the provision of call coverage.
