Directions: The purpose of the activity tracking chart is to help you keep track of the number of activities you complete during the Get Fit Challenge. You may keep
notes on this chart, the printed chart that was mailed to you in your Tap into Wellness packet or anywhere else that is convenient for you. This is for your own
personal use and should not be turned in with your challenge results . You may record the number of activities completed each day, or a description of each
activity, or both. At the end of the challenge, add up the total number of healthy eating activities and the total number of physical activities you completed. Turn those
two numbers in (with your beginning and ending weight, if you're participating in the optional weight loss contest) on your postcard or the online submission form at
www.rowanwellness.com (login: rowan; password: wellness).
