64 - PERFORMANCE

64.2 - Corrective Actions
Government driving privileges shall be immediately revoked for any of the following:

        1. Conviction of operating a motor vehicle while under the influence of alcohol,
narcotics, or pathogenic drugs. (FPM Ch. 930)

       2. Conviction of leaving the scene of an accident in which the operator is involved
without making himself/herself known. (FPM Ch. 930)

       3. Failure to meet required physical standards as set forth in State and Federal Laws and
Regulations. (FPM Ch. 930)

         4. Revocation or suspension of state driver's license. (FSH 6709.11)

       5. Violation of any Laws and/or Administrative Regulations relating to motor vehicle
operation. (FSH 6709.11)

         6. Improper, negligent or abusive use of the vehicle and/or equipment. (FSH 6709.11)

       7. If the operator's driving record shows two (2) vehicle accidents for which the operator
was determined to be at fault and/or three (3) moving (traffic) violations during the preceding
four years.

       8. Failure to attend a defensive driving course or refresher during the preceding four
years. This requirement may be waived for new employees and temporaries until the next
scheduled course. (FSH 6709.11, FPM Ch. 930)

       9. Failure to report any moving violations to the immediate supervisor and the
Driver/Operator Program Manager within 30 days. Any driver's license revocation shall be
reported by close of business on the following workday.

      10. Any accident, regardless of fault, while operating a Government-
owned/rented/leased vehicle. (FSH 6709.11)

Reinstatement of driving privileges shall be at the discretion of the Fleet Manager and/or
Driver/Operator Program Manager following a review of individual circumstances.
