                                            METHOD OF PAYMENT
                           Make all registration fees payable to Norma’s House
 A single check or money order may be used for multiple registrants; however, please write the names of all
 registrants on the form of payment and include an individual registration form for each attendee. Written
requests for refunds will be honored if postmarked no later than April 1, 2008, however, a $10.00 handling fee
                                        will be deducted per registrant.
