PARENT/PARISH COVENANT

							       RELIGIOUS EDUCATION REGISTRATION FEE AND
                    REQUIREMENTS

                     For Grades EC3 through Grade 8


Thank you for your timely attention to registration requirements! They are:


1.    The Registration form. Please fill out a separate form for each child.
Please notice and fill out the Acknowledgements page.

2.    A copy of each child’s Baptismal Certificate. If a record of baptism was
previously presented to the RE office, you do not need to submit another.
3.    Early Registration fee (from April 1 to July 1):
$50.00 for the first student in the family; $45.00 for each additional student.

4.    Registration fee (after July 1):
$60.00 for the first student in the family; $55.00 for each additional student.




                            Registration Checklist:
                                Have you included:


                  Separate registration form for each child

                  Baptismal certificate copy for each new student(s)

                  Signed Acknowledgements form

                  Registration fee (check payable to “Christ the King Church”
