CERTIFICATE OF INCORPORATION
We, the subscribers, pursuant to and in conformity with an Act of the Legislature of the
State of New York, entitled “The Religious Corporations Law,” being chapter 53 of the
laws of 1909, and the several acts amendatory thereof and supplemental thereto, do
declare and certify:
That a notice of meeting for incorporating the unincorporated Congregation of
__________________________Baptist Church, whose place of worship is located in the

State of New York, was duly conspicuously posted on the outside door of the main
entrance to our place of worship on __________, 20___, and duly publicly read at the
