8. The applicant must attach four Schedules as part of this application:
(a) Schedule A must contain one or more declarations signed by persons with first
hand knowledge, detailing all of the facts upon which the applicant relies. Each
signed declaration must include the following statement: “This declaration has
been prepared by me or under my instruction and I hereby confirm its accuracy”.
(Please note that the applicant will not be allowed to present evidence or make
any representations about any material fact that was not set out in the application
and filed promptly in the way required by the Board's Rules of Procedure, except
with the permission of the Board.)
(b) Schedule B must contain a list of all documents on which the applicant intends to
rely. A copy of those documents, if the applicant has them, must be attached to

by the parties and a list of those issues that remain in dispute.
(d) Schedule D must contain a proposed collective agreement that the applicant is
prepared to sign.
