      1. Bio data of the candidates to be submitted duly signed by the candidate.

      2. Original documents along with self signed Xerox copies to be submitted at the time of interview.

          The original certificate will be returned after verification.

      3. Registration for attending Interview will be made within 10.00AM to 11.30 AMon the date fixed
         for walk-in-interview. No candidate will be allowed to register their name for interview beyond
         the specified time.

      4. No TA & DA will be allowed for attending Interview.

      5. ORV ACT will be applicable as per Govt. Rules prevailing at the time of selection.

      AGE:        Must be above 18 yrs & maximum 32 yrs at the time of date of walk-in-interview.
      Relaxation of age for ST/SC as per Government norm.

      MODE OF SELECTION:

1.    The Selection will be made strictly on the basis of merit on the marks secured by the candidate in
      the qualifying examination (essential qualification) prescribed for the post. No weight-age shall be
      given for candidate having B.Sc.(Nursing) in case of Staff Nurse.
2. GENERAL CONDITION:
  1. The engagement is purely on Contractual basis & temporary & may be terminated at any time
     without Notice & without assigning any reason thereof.

  2. No. of Posts under different category may vary as per norm / vacancy position at the time of
     selection.

  3. The candidates cannot claim any regular appointment in future as a matter of right & also will
     not claim Inter-Dist. transfer. They will not claim any financial benefit other than the monthly
     consolidated remuneration & other allowances as per State Govt. Rules.

  4.   They will not claim any Govt. Accommodation.

  5. Preference will be given to the candidates belonging to Balasore District. In case sufficient
     candidate not available in Balasore District, applicants from immediate neighbour district Will be
     considered.

  6. Candidates passed from Govt. Institutions will be given preference.

  7. The undersigned reserves the right for modification or even cancellation of the advertisement
     without assigning any reason thereof.

  DOCUMENTS TO BE SUBMITTED AT THE TIME OF WALK-IN-INTERVIEW:
  a. Attested copies of educational certificate & mark sheet from HSC Examination onwards along
     with the proof of Date of Birth.

  b. Attested copies of Caste Certificate obtained from the competent authority.

  c. Attested copies Residence / Nativity Certificate obtained from the competent authority. 6 (six)
     months validity must be intact by the time of interview.

  d. Three recent Passport Size Photograph duly attested by the Gazetted Officer.

  e. Attested copy of Registration Certificate from the concerned Council.

  f.   All documents must be self attested.
