I understand that completion of this form does not guarantee membership, but that I am being considered for
membership into the National Honor Society. I will accept the decision of the faculty committee charged with the
responsibility of selecting members as final. I know I can apply again next year. If I become a member, I do not
need to reapply each year, but only have to pay my yearly $5 dues to maintain membership in NHS.

                            Membership is based on the following criteria:

 NHS is more than just an honor roll. The Honor Society chapter establishes rules for membership that are based
 upon a student's outstanding performance in the areas of scholarship, service, leadership, and character. These
 criteria for selection form the foundation upon which the organization and its activities are built.

 Scholarship: Students who have a cumulative grade point average of 85 percent, B, 3.0 (on a 4.0 scale) or
 equivalent standard of excellence, or a higher cumulative average set by the local school's Faculty Council, meet
 the scholarship requirement for membership. These students are then eligible for consideration on the basis of
 service, leadership, and character (and citizenship for NJHS).

 Service: This quality is defined through the voluntary contributions made by a student to the school or
 community, done without compensation and with a positive, courteous, and enthusiastic spirit.

 Leadership: Student leaders are those who are resourceful, good problem solvers, promoters of school activities,
 idea-contributors, dependable, and persons who exemplify positive attitudes about life. Leadership experiences
 can be drawn from school or community activities while working with or for others.

 Character: The student of good character upholds principles of morality and ethics, is cooperative, demonstrates
 high standards of honesty and reliability, shows courtesy, concern, and respect for others.       
                                   ACTIVITY SHEET
1. First, clearly and thoroughly list ALL the activities (in school and out of school, clubs,
athletic teams, jobs, youth groups, church groups, etc.) that you have been involved in
during your high school years and a description of what role you have played in that
activity. Please be thorough in your description and be as specific as possible. Then,
highlight the volunteer and leadership activities.
2. Volunteer-Choose a color and highlight any volunteer service you have done in those
activities (for example: beach clean-up one Saturday each month with Key Club)
3. Leadership-In another color, highlight any activities in which you’ve demonstrated
leadership (for example: president, secretary, captain, manager, leader, etc.).
4. We are looking for at least 2 examples of volunteer service and at least 2 examples of
leadership from each grade level.
5. Please thoroughly explain and elaborate on these activities. Be as specific and detailed as
possible. You may continue onto the back of this sheet or attach another sheet.
