Case of the week – 06-10: Myocarditis
History: A 42 year-old ♂ admitted with 3 days of chest pain and diarrhoea
ECG/enzymes: Inferior ST-elevation, CK 597IU, Troponin I 13.65ng/ml
CMR: Increased intensity on STIR (Inversion recovery sequence with short
TI) not shown. Patchy enhancement after Gadolinium.
Immunology: ↑antibodies against Campylobacter jejunii
Treatment: Full resolution after 7 days antibiotics
Follow-up: At 6 months STIR normalized, late
Perspective: CMR may be used
to diagnose acute myocarditis,
guiding medical therapy.
