Permanent Housing Program
Title: Case Manager
Salary Range: DOE
Hours of Work: 40 hours weekly
Reports to: Program Manager
The Case Manager is responsible for brokering access to in-house and external services for
families with the overall goal of transitioning the families to permanent housing and self-
sufficiency. The Case Manager will also assist with assigned administrative responsibilities,
resource development and networking, participating in trainings and staff meetings.
A Bachelors Degree in Human Services or related field (preferably working with
adolescents/young adults), with a minimum of 3 years case management experience.
Duties and Responsibilities:
In partnership with family members, create (and continuously update)
comprehensive service plans.
Provides general administrative assistance to the Program Manager
Assists with completing monthly reports.
Provide orientation to new SBY staff; assist with orientation of new staff and clients.
Acts as the lead in the absence of the Program Manager; and oversees the house
budget pertaining to program expenditures.
Assist with planning, preparation and implementation of other activities of program
Assist the Program Manager with the development and maintenance of
administrative systems to facilitate and enhance the delivery of services. Complete
assigned administrative responsibilities in a timely manner.
Coordinate and participate in special projects.
Assist with completing monthly staff schedule and activity schedule. Assist with
locating emergency coverage.
Participate in interviews of potential employee applicants.
Provide ongoing, hands-on assistance to residents in preparing for—and securing—
Oversee the reporting, planning and implementation of activity schedules.
Arrange transportation to in-house and alternative services as needed.
Assist residents in gaining workplace skills and in accessing job training, internships
Provide financial management education.
Plan, schedule, organize and supervise recreational activities.
Carry out emergency fire evacuation, and emergency procedures in case of an illness
Provides office coverage, answer and record all phone calls and visitors, enforce
“house rules” and ensure safety of the transitional living facilities.
Participate in training provided or recommended by the Agency.
Participate in regular supervision with the Clinical Director and the Program
Attend and participate in monthly staff meetings and other meetings/activities
identified by the program manager/director.
Provide appropriate referrals; provide individual and group counseling, as needed.
Follow-up on referrals made and report efficacy of referrals as required.
Coordinate the involvement of other agencies and/or personnel involved with
youth and young adults residing in the program’s residences.
Other duties as assigned by the Program Manager.
