VII. STATEMENT OF APPLICATION AND CERTIFICATION

    A. An Application Fee in the amount of $         350.00       is included herewith and I understand that it is earned by the Mississippi Home
       Corporation (MHC) upon receipt of this application and is nonrefundable whether or not a commitment is issued by MHC.


    B. To the best of my knowledge and belief, no information contained in this application is in any way false or incorrect; that it is truly
       descriptive of the development or property for which this Application is submitted applied; and the proposed construction will not violate
       zoning ordinances or deed restrictions.

       I hereby make application to Mississippi Home Corporation (MHC). I agree that neither MHC nor any of its directors, officers, employees,
       and agents will be held responsible or liable for any representations made to the undersigned or its investors relating to MHC; therefore I
       assume the risk of all damages, losses, costs, and expenses related thereto and agree to indemnify and hold harmless the Mississippi
       Home Corporation (MHC) and/or any of its directors, officers, employees, and agents against any and all claims, suits, losses, damages,
       costs and expenses of any kind, and of any nature that the Mississippi Home Corporation (MHC) may suffer, incur, or pay arising out of or
       relating to its decision concerning this application.

       I agree that Mississippi Home Corporation and/or MHC may request any additional information, which at the discretion of Mississippi
       Home Corporation and/or MHC is necessary or appropriate to its evaluation of this application.

    C. This application is filed in accordance with the original development cost and financing information provided in the original tax credit
       application filed with MHC on

    D. I DO HEREBY CERTIFY THAT THE ABOVE INFORMATION AND ANY ATTACHMENTS IN SUPPORT THEREOF ARE TRUE,
       ACCURATE AND COMPLETE. I UNDERSTAND THAT ANY MISREPRESENTATION IN THIS APPLICATION OR SUPPORTING
       DOCUMENTATION MAY RESULT IN THE APPLICANT NOT BEING CONSIDERED FOR APPROVAL OF THIS APPLICATION BY
       THE MISSISSIPPI HOME CORPORATION AND THE MISSISSIPPI DEVELOPMENT AUTHORITY (MDA), AND THE APPLICANT AND
       RELATED PARTIES BEING BARRED FROM FUTURE PROGRAM PARTICIPATION.

Please attach a copy of the Phase I Environmental Site Assessment for the development, if available.

This preliminary environmental checklist is designed to make the applicant aware of major environmental concerns that
should be addressed in project planning and budgeting. Each successful applicant will be required to prepare an
environmental review record, and if appropriate, an environmental assessment in accordance witht he National
Environmental Policy Act of 1969, as amended. The environmental assessment involves an examination of the
environmental conditions and circumstances that exist at the location of the project and a determination of project impact
on those environmental conditions. In addition, each project must be reviewed for compliance with other applicable
environmental laws and regulations. If any of the following questions elicit an affirmative response, additional
information must be provided as part of the environmental review.
