                                   Section I: Performance Rating Definitions
There are four ratings that XYZ uses to measure employee performance. Although XYZ does not have a “forced
performance distribution” of employees by performance rating, it is expected that the overall ratings of the organization
will reflect a broad spectrum of defined performance levels. On a company-wide basis XYZ expects that its distribution
of these four ratings would be similar to industry standards. These standards suggest that approximately 20% of XYZ
employees would be rated a level 4 performance rating, 30% a level 3 rating, 45% a level 2, and 5% would be rated a level
1 performance rating. Use the following performance rating categories and definitions to evaluate employees:

Level 4 (Consistently Exceeds Expectations)
Performance consistently exceeds expectations in the major job responsibilities and performance categories. The employee’s
judgment, decision making, and knowledge are well respected. Supervisors and co-workers generally seek the employee’s
support, advice and direction regarding projects. The employee is a good candidate for expanded responsibilities and adds real
value to XYZ.

Level 3 (Meets and Sometimes Exceeds Expectations)
Performance meets expectations in the major job responsibilities and performance categories, and exceeds expectations in some
areas. The employee is a solid performer who produces results and works well in a team environment. The employee is an active
and valuable member of the XYZ team who strives to find ways to improve their own skills and contribute to the success of XYZ.

Level 2 (Meets Most Expectations)
Performance meets most or all expectations in the major job responsibilities and the performance categories. The employee
performs their job well. The employee should work to expand their skill set and enhance their performance

Level 1 (Rarely Meets Expectations)
Inconsistently meets or does not meet the requirements of their job. An improvement plan must be established for
employees receiving this rating. If performance does not improve to a level 2 performance within 3 months, further
disciplinary action, up to and including termination of employment, will be taken.

                                        Section II: Performance Categories
The following performance categories are identified as being key to an employee’s success at XYZ. They also represent
the values that XYZ believes are key competencies and values that will make XYZ successful. Each category contains
example items that are considered when evaluating employee performance. Following the examples are the supervisor’s
