An exciting opportunity has arisen for an experienced Advocacy and
Support Worker, Care Manager, qualified Social Worker or Health
Professional to work on a pilot project to provide a Care Management
service to self funders in Bournemouth.
The aim of the service is: To provide to self-funders a Care Management
service that offers them information, advice, support planning and brokerage.
This will enable individuals to make well informed choices and create a
support plan that provides solutions to their needs in a personalised way.
The Care Advisor who would provide face to face individual and personalised
assessment and support planning to self funders requesting intensive
assistance in addressing their needs. The Care Advisor would either be a
qualified Social Worker, an experienced Care Manager, Advocacy and
Support Worker or Health Professional (supervised by a qualified Social
Worker currently employed by Help and Care).
The Care Advisor would listen to the client and undertake a supported self
assessment using Tools and Techniques of Person Centred Planning to
identify their needs and preferences. They would then assist that person to
create a Support Plan to set out the different ways the client wishes to find
solutions to meeting their needs. The Support Plan would address all aspects
of the client’s life including relationships and community networks, aspirations,
wellbeing, hobbies and socialising, citizenship and communication as well as
the practical elements of housing, personal care, health care and
The Care Advisor would work closely with the client to put into place the range
of support solutions and agree a schedule for review and monitoring. It is
anticipated that, on average, the initial assessment/care planning would
require 4-6 hours and within a 12 month period, three further reviews of 2
hours each. The service will need to test out this assumption. Self funders’
support needs will vary enormously. This is acknowledging that some clients
would require much less input and some much more.
