JOB PURPOSE
The postholder is responsible for assisting the Leg Ulcer nurse/specialist for patients
presenting with lower leg conditions, and the provision of leg ulcer treatment & preventative
nursing activity to all the population registered with GP’s in South Somerset. To work as an
effective and responsible team member
CONTEXT
The post holder works as part of the Hendford Lodge Group that is responsible for providing
expert nursing care, within community Practice/hospital premises, to the whole of the
practice population of South Somerset. S/he will be responsible for the care they deliver,
both autonomously and as part of a team. Sensitive information relating to patients health will
be gathered and divulged in the strictest of confidence, and the patient has the right to
expect respect and privacy and for the nurse to act appropriately at all times. Confidential
information relating to patients and other colleagues is to be regarded in the strictest
confidence at all times
CLINICAL RESPONSIBILITIES
Ensuring high standards of care is delivered to patients within the leg ulcer service
To undertake clinical tasks of leg care taught by the trained nurse
Demonstrate an understanding of leg ulcer management
To be aware of and follow infection control practices within the clinic environment
Provide support to patients/relatives, carers and staff
To encourage the patient towards self-care and independence
To observe and report changes in the patients condition, be that physical, mental
and/or social to the trained nurse on duty
Ensure detailed, accurate and confidential patient records are maintained whilst
adhering to policies

KNOWLEDGE & SKILLS
Be committed to working in a changing environment
Manage and assess risk within areas of responsibility, ensuring adequate measures
are in place for protecting patients and staff
Establish and maintain effective communication with patients & colleagues
Work closely and communicate effectively with other members of the health care
team; including nurses and patients maintaining confidentiality at all times
To undertake arrangements for patients to attend the Leg Ulcer clinic from referral
letters
Have basic IT skills and contribute to organisation data collection
To attend all mandatory training provided by the organisation
Identify own training needs to acknowledge own limitations
Participate in the appraisal system
To be proactive in developing own skills and knowledge
Undertake such other duties as may be required from time to time and are consistent
with the responsibilities of the grade.

PERFORMANCE MANAGEMENT
Initially the post holder will be reviewed at 3 and 6 months. Thereafter, s/he will participate in
an annual performance appraisal and mutually agree a Personal Development Plan (PDP)
