									Job Description for Cardiology and CCU Components -CMT Training, Lincoln
County Hospital.


Summary of Learning Objectives

1.     To gain an appreciation of the range of cardiological conditions managed in
       secondary care
2.     To gain particular experience of the management of patients with acute
       coronary syndrome, ischaemic heart disease and heart failure.
3.     To gain experience in the management of common arrhythmias.
4.     To be proficient in the performance and interpretation of ECG.
5.     To gain experience in the appropriate use and interpretation of
       Echocardiography and angiography.
6.     To gain an understanding of the indications for and complications of
       permanent pacemaker implantation.


The Firm
The SHOs working on the Cardiology Firm see the full spectrum of cardiac
pathology. There are full diagnostic facilities including echocardiography, ambulatory
monitoring, cardiac nuclear medicine and cardiac catheter laboratory. Apart from
medical staff there are 3 specialist nurses and 6 cardiac technicians. In-patient work is
based on Johnson ward and the Coronary care Unit. There are 6 cardiology clinics per
week.
The Team
Consultants: Dr R Andrews
             Dr S Kelly
             Dr W Arthur

The Department’s inpatients are spilt between the three consultants. SHOs are
nominally attached to Dr Andrews and Dr Kelly with the remaining SHO attached to
CCU.
Consultant cover for the coronary care unit is provided on a rota basis with a daily
CCU ward round.

There are two cardiology SpRs and a staff grade.

* at present the core teaching rotates through days from Monday pm – Friday am –
with CMT this will be fixed to a morning which may be Wednesday.

Achievement of Learning Objectives
To gain an appreciation of the range of cardiological conditions managed in
secondary care Attendance at the general cardiology clinics, together with inpatient
based work on the cardiology ward will expose the trainee to the variety of
cardiological conditions.

To gain particular experience of the management of patients with acute coronary
syndrome, ischaemic heart disease and heart failure. – the trainee will gain
experience of the management of acute coronary syndromes through work in the
inpatient setting and when on call. Experience of more general management of
ischaemic heart disease and heart failure will be gained by both experience in the
ward setting and attendance at the general cardiology clinics where both new and
follow up patients will be seen under consultant supervision. Attendance at rapid
access chest pain clinic will allow experience to be gained in outpatient assessment of
chest pain.

To gain experience in the management of common arrhythmias. – experience of the
investigation and management of common arrhythmias will be gained in the inpatient
and outpatient setting. By the end of the attachment the trainee will have
demonstrated an understanding of the indications for both elective and emergency
cardioversion and competency in its administration.

To be proficient in the performance and interpretation of ECG. - Training in the
interpretation of ECG’s will be given throughout the attachment.

To gain experience in the appropriate use and interpretation of Echocardiography
and angiography. – experience in the appropriate use of all cardiological
investigations will be gained during the attachment. There will be an opportunity to
spend sessions in both the Echocardiography lab and cardiac catheterisation suite.
Participation in the departmental echocardiography meeting will further trainees skills
in interpretation.

To gain an understanding of the indications for and complications of permanent
pacemaker implantation. The trainee will gain an understanding of the indications for
permanent pacemaker implantation (and the complications of the procedure both short
term and longer term) from attendance at outpatients and through involvement of
patients with arrhythmias on the cardiology ward.


Other Learning Opportunities
There are opportunities to observe procedures such as cardiac catheterisation, Trans-
oesophageal echocardiography, temporary and permanent pacing and Stress ECHO.
