Get With The Guidelines is a hospital based program that focuses on care team protocols
designed to maximize patient adherence prior to discharge on appropriate medications and risk
Program Purpose: The American Heart Association impact goal is to reduce coronary heart
disease, stroke and risk by 25% by 2010. The Get With The Guidelines-CAD program will
assist health professionals in institutions implementing the American Heart Association/
American College of Cardiology Prevention Guidelines in patients discharged with coronary
artery disease (CAD). Implementing the Get With The Guidelines-CAD program nationwide can
save as many as 80,000 lives per year.
GET WITH THE GUIDELINESSM
Implementing the American Heart Association (AHA) and American College of Cardiology
(ACC) Secondary Prevention Guidelines in patients discharged with coronary artery
Understand how to integrate current scientific guidelines into clinical practice for
Understand the concept of Rapid Cycle Quality Improvement (RCQI) and how it is
employed to promote multidisciplinary change in the acute care setting.
Learn techniques and strategies to address common implementation challenges
Integrate key learnings and develop an Action Plan designed to accelerate
adherence to guidelines.
CME and CEU credits are offered for participating in this workshop.
Deadline for pre-workshop registration is 10 days before workshop-March 19, 2004 on
site registration will be available at the workshop

Continuing Medical Education Accreditation — Physicians
The American Heart Association is accredited by the Accreditation Council for Continuing Medical
Education to provide continuing medical education for physicians.
The American Heart Association designates this educational activity for a maximum of 4.50 category 1
credits toward the AMA Physician’s Recognition Award. Each physician should claim only those credits
that he/she actually spent in the activity.
All faculty participating in CME/CE activities sponsored by The American Heart Association will disclose
to the audience (1) significant financial relationships with the manufacturer(s) of products from the
commercial supporter(s) and /or the manufacturer(s) of products or devices discussed in their
presentation, and (2) unlabeled/unapproved uses of drugs or devices discussed in their presentation.

AACN guidelines for 5.0 contact hours, CERP Category A.

The American Heart Association is accredited by the Accreditation Council for Pharmacy
Education as a Provider of continuing pharmaceutical education.
ACPE Credit: 4.50 Contact Hours or 0.450 CEUs. Universal Program Number: 256-000-
Successful completion of this CE activity includes the following: (1) Attending the program lecture
sessions and a concurrent panel session. (2) Complete the appropriate CE request application and
activity evaluation form. (3) Submit the forms to the on-site AHA staff for processing. Participants will
receive CE certificate of credit within 6 weeks following receipt of the completed CE request application
and activity evaluation form. There is no fee for CE credits for this activity.

The GWTG program offers you an easy solution to implementing rapid cycle quality improvement in
coronary heart disease through an easy to use Web-based tool. The American Heart Association’s
Patient Management Tool is an online, interactive assessment and reporting system that helps you
implement the GWTG program. It does this by providing patient-specific AHA guidelines information
and enabling each institution to track its adherence to the guidelines individually and against the
AHA’s national benchmarks over time. This Web-based tool is an important part of implementing
