                      MAGNETIC RESONANCE IMAGING (MRI) GUIDELINES

Your medical provider has determined you need magnetic resonance imaging (MRI). This guide
provides you with available resources and information on scheduling an MRI.
Please go directly to the Radiology Department to complete a MRI questionnaire. This can be
completed at the Radiology Department at the Naval Hospital, Naval Branch Health Clinic (NBHC)
Jacksonville or NBHC Mayport. Your appointment at the hospital cannot be scheduled until the
questionnaire has been completed. Once the MRI request has been reviewed and approved by a
radiologist, the MRI scheduler will contact you within five working days to schedule an appointment
for your procedure. If you have not been contacted by the MRI scheduler, please call (904) 542-9693.
Active Duty Service Members – MRIs for active duty personnel will usually be provided at the Naval
Hospital. If the care is not available, you will be referred to the civilian network. Your network
referral will be handled by the Health Benefits Center. You will be contacted within five business days
to schedule your appointment. For more information or to speak with a Health Benefits Advisor, call
(904) 542-9164. Please ensure your information is correct in the Defense Enrollment Eligibility
Reporting System better known as DEERS. (See Note).

TRICARE Prime Active Duty Family Members, Retirees and Their Families Enrolled at Naval
Hospital Jacksonville
If the Naval Hospital can provide your study, the MRI scheduler will contact you within five
business days to schedule an appointment. If the care is not available at the hospital, you will be
referred to the TRICARE network.
You will be notified within 7-10 business days if your referral has been sent to the network. If you
have not been notified after 10 days, please contact Referrals Management at 904-542-9088 or 9099.
The staff will be happy to answer any questions you have regarding the referral process.

If referred to the TRICARE network, you will receive an authorization in the mail from the
TRICARE Contractor that will include the provider's name, address, telephone number, number of
visits authorized, expiration date and the authorization number. Please schedule your MRI
appointment as soon as possible and bring this letter with you to your appointment.
All Patients – Once the procedure is completed, request a copy of the MRI scan and disk. Please
bring the MRI scan to your follow-up appointment to ensure the results are available for your
provider, otherwise, your appointment may be rescheduled. Please take the MRI disk to the
Naval Hospital Radiology Department to be uploaded into your electronic medical record.
If the service is provided in the civilian network, please ask the provider to fax your MRI report to the
Naval Hospital’s Referrals Management Office at (904) 542-9709.
