INVITATION TO BID
Sealed bids marked "AMBULANCE SERVICE -COOPER GREEN HOSPITAL/FAMILY COURT
and JEFFERSON REHABILITATION AND HEALTH CENTER" will be received by the
Purchasing Manager, Room 820 Courthouse; Birmingham, Alabama.
Bids will be accepted until 5:00 PM central time (standard or daylight savings time, as applicable) on
JULY 19, 2006. Bids submitted after this date and time will not be considered.
Bids will be publicly opened at 2:00 P.M. JULY 20, 2006.
The County is not responsible for delays occasioned by the U.S. Postal Service, the internal mail delivery
system of the County, or any other means of delivery employed by the bidder. Similarly, the County is
not responsible for, and will not open, any bid/proposal responses, which are received later than the date
and time, indicated above. Late bids/proposals will be retained in the bid/proposal file, unopened.
CONDITIONS
Contract will be good for the period 10/1/06 to 10/1/07 with renewal, at County’s option, two additional
years, renewing each subsequent fiscal year on October 1st, and ending 10/1/2009.
SPECIFICATIONS:
VENDOR MUST HAVE THE FOLLOWING:
A maximum response time of 10-15 minutes with backup of no less than 10 units with a 10 – 15
minute response time when primary unit is on a call.
On staff Medical Director with continuous review and updates to ensure all state, county, and
local clinical protocols are followed.
Availability of Mobile Intensive Care Units for Critical Care transports to ensure adequate
equipment and personnel for the most critically ill.
Liability insurance primary coverage of at least $1,000,000 with excess of not less that
$5,000,000 in aggregate to reduce the institution or local government liability.
A Comprehensive Driver Training Program to ensure the safety of the patient being transported
and the citizens at large.

Continuing Medical Education and OSHA Training to its employees to ensure the highest level of
clinical expertise and working environment safety.
Each vehicle equipped with 12 lead EKG with transmit capability.
Computer Aided Dispatch System staffed by EMD Certified Dispatchers to ensure timely
response and appropriate record keeping to enhance the integrity of performance reports.
Quality Assurance and Quality Improvement Department to ensure that quality is continually
maintained and improve patient care.
Documented Community Programs that the provider participates in for the privilege of providing
service to the institutions or local government.
PERSONNEL REQUIREMENTS:
Two (2) each employees for these services shall meet the requirements for ambulance operators
as set by the City of Birmingham, the Emergency Medical Service of the State of Alabama and/
the Federal Department of Transportation.
Qualified personnel will be immediately available to provide this service for both routine and
emergency transportation as specified for Cooper Green Hospital, Jefferson Rehabilitation &
Health Center and Family Court.
Qualified personnel must attend a comprehensive driving program on a biennial basis and shall be
monitored for good driving and safety practices. Proof of said training must be sent to Jefferson
County after each course.
All EMT/Paramedics must have current Basic Cardiac/Life Support/ Cardiopulmonary
Resuscitation Certification through the American Heart Association

Time of performance and quality of performance are of the essence of this contract. Failure to
perform within the guideline of the contract will constitute sufficient grounds for cancellation at
the option of the county commission. Said contract may be cancelled with 30 day written notice.
Transportation needs:
Patients will be picked up by qualified persons from their assigned rooms by wheelchair or
stretcher and transported to the specified destination.
Transport needs will include Transportation of ambulatory/ wheelchair, stretcher and critically ill
patients.
All supplies necessary for transport or patient care during transport shall be supplied by vendor.
All necessary emergency equipment and supplies will be maintained in the transport vehicle at all
times.
Vehicle Total shall be a number deemed sufficient by the Cooper Green Hospital Medical Staff,
Jefferson Rehabilitation and Health Center and Family Court and the Jefferson County
Commission to assure successful performance of any contract awarded as a result of this bid.

Any and all subcontracts entered into by the successful bidder as a result of this bid are subject to
approval by the Jefferson County commission. A listing of these contracts shall be furnished to
the Jefferson County Purchasing Department no later than 7 days after said contract commences.
By accepting the above listing, Jefferson County does not assume responsibility for payment to
said subcontractors. The responsibility for payment lies with the successful bidder.
Permits, Codes & Regulations: All vehicles, equipment and service rendered must comply with city
county and state codes and regulations. Said compliance must be in effect at the time of the bid opening.
Proof of said compliance must be submitted 3 days after request by the county commission.

Reimbursement:
The successful bidder will accept assignment for transport of all patients eligible for third party
reimbursement.
In the event of general price reduction, the County will receive the benefit of such reduction on
any undelivered portion of contract.
Due to bid evaluation and budgetary procedures, bid awards will not be made prior to October 1, 2006. At that time
a Bid Award Notice and Contract Order will be issued to successful bidder.
INVOICING
All invoices must agree with the purchase order in description and price and include the following
information: 1) Purchase Order Number; 2) Ship-to department name and address.
In order to ensure prompt payment, ALL ORIGINAL INVOICES* MUST BE SENT TO:

*If invoice does not agree with purchase order, credits or a corrected invoice will be required in order for
the county to process payment.
THIRD-PARTY “REMIT-TO”:
If Bidder has a third-party “remit-to” company, that information must appear on the Bidder’s response.
Jefferson County will send payment to the company designated by Bidder on its response, but will not be
responsible for resolving payment issues, should the Bidder change payment processing companies after a
payment has been mailed or without 45-day written notification to the Purchasing and Accounting
division of Jefferson County.
PRE-PAYMENTS:
No prepayments of any kind will be made prior to transport.
TAX: Jefferson County is exempt from all tax. Provided however, bidder shall be responsible for
payment of all sales, use, lease, ad valorem and any other tax that may be levied or assessed by reason of
this transaction.

If mutually agreed upon, additional contracts may be executed, based upon this bid for the same service(s)
or related services and locations.
Bidder agrees that Jefferson County will be charged no more for item(s) bid than the State of Alabama,
and that in the event of a price reduction; the County will receive the benefit of such reduction on any undelivered
portion of contract.
GUARANTEE: Bidder certifies by bidding, that he is fully aware of the conditions of service and purpose for
which item(s) included in this bid are to be purchased, and that his offering will meet these requirements of service
and purpose to the satisfaction of the Jefferson County Commission and its Agent.
INSURANCE: Contractor will maintain such insurance as will protect him and the County from claim under
Workmen's Compensation Acts, and from claims for damage and/or personal injury, including death, which may
arise from operations under this contract. Insurance will be written by companies authorized to do business in
Jefferson County, Alabama. Evidence of insurance will be furnished to the Purchasing Agent not later than 7 day(s)
after Purchase Order date.
The successful bidder will maintain such insurance as will protect him and the County from claim under Workmen's

Insurance Minimum Coverage:
Contracting party shall file the following insurance coverage and limits of liability with the County's Risk
Management Office and Purchasing Department before beginning work with the County.
General Liability:
$1,000,000 - Bodily injury and property damage combined occurrence
$1,000,000 - Bodily injury and property damage combined aggregate
$1,000,000 - Personal injury aggregate
Comprehensive Form including Premises/Operation, Products/Completed Operations, Contractual,
Independent contractors, Broad Form property damage and personal injury.
Automobile Liability:
$1,000,000 - Bodily injury and property damage combined coverage
Any automobile including hired and non-owned vehicles

GENERAL: The Jefferson County Commission expressly reserves the right to reject any or all bids, or
parts of bids, and to make the award or awards as the best interest of the County appears.
BID BOND: Each bidder will furnish a bid bond with their bid response, or cashier’s check in the
amount of $5000.00, and hereby agrees that in case of failure to execute a contract and furnish a
performance bond within 14 days after notice of award, the Jefferson County Commission will be paid by
this bonding company the difference between the amount of the low bid as submitted, and the amount of
the proposal of the next higher bid, or if no other bid is received, the full amount of the proposal guaranty
will be recovered as liquidated damages for such default.
PERFORMANCE BOND: Successful bidder will furnish a surety bond in the amount of the contract at
no cost to the County, to guarantee that all conditions and specifications of this Invitation will be fulfilled.
Bond will be furnished to the Purchasing Agent not later than 14 days after requested.
